摘要
目的:探究帕金森病(PD)患者的生态执行功能特征。方法:招募33例符合《中国帕金森病的诊断标准(2016版)》的PD患者及35名认知功能正常中老年人,采用中文版10项周历活动规划(WCPA-10)评估生态执行功能,分析PD组与认知正常组WCPA-10指标结果的差异以及PD组WCPA-10指标间的相关性。结果:PD组WCPA-10的效率、准确填入的安排数、规则遵守、错误类型、时间意识和准确性估计得分低于对照组(均P<0.05),PD组总时间与计划时间2呈高度正相关(r=0.72,P<0.01),填入的安排数与准确填入的安排数、效率与总时间、计划时间1与计划时间2、计划时间1与总时间、策略使用数与准确填入的安排数之间呈中度正相关(r>0.52,均P<0.01)。结论:PD患者较认知功能正常的中老年人存在生态执行功能减退,受累的认知成分包括抑制控制、工作记忆及自我意识。
Objectives:To investigate the characteristics of ecological executive function in patients with Parkinson′s disease(PD).Methods:Thirty-three patients who were diagnosed as PD according to the Diagnostic Criteria of Parkinson′s Disease in China(2016) and 35 elder individuals without cognitive impairment were recruited.The Chinese version of 10-Item Weekly Calendar Planning Activity(WCPA-10) was conducted to evaluate ecological executive function.Between-group variance and correlation between indices of WCPA-10 in PD group was analyzed.Results:In WCPA-10,the performance on efficiency, accurate entered appointment, followed rules, error type, time awareness, accuracy estimation were significantly worse in PD group than in normal cognition group(P<0.05).Total time and planning time 2 were highly correlated(r=0.72,P<0.01),moderate correlation was found between entered appointment and accurate entered appointment, efficiency and total time, planning time 1 and 2,planning time 1 and total time, strategy use and accurate entered appointment(r>0.52,Ps<0.01).Conclusion:Patients with Parkinson′s disease may have significant deficits in ecological executive function compared with elder adults without cognitive impairment.The involved cognitive components are inhibitory control, working memory, and self-awareness.
