三维超声测量孕中晚期胎儿肾上腺体积的正常参考值范围 被引量:3

Reference Ranges of Fetal Adrenal Gland Volumes in the Middle and Late Trimester by ThreeDimensional Ultrasound

摘要 目的观察孕中晚期正常胎儿肾上腺体积与孕周、胎儿体重之间的相关性,并确定正常参考值范围。资料与方法使用三维超声前瞻性获取2021年3—12月中南大学湘雅二医院行胎儿超声检查的21~38+6周正常单胎妊娠胎儿肾上腺容积图像,并使用VOCAL法测量肾上腺体积,拟合多种回归模型,建立各孕周胎儿肾上腺体积正常值范围,探讨胎儿肾上腺体积与胎儿体重的关系。结果成功测量590例正常胎儿的肾上腺体积,基于孕周最佳回归模型为二次多项式(R^(2)=0.896,P<0.001),建立各孕周胎儿肾上腺体积的各百分位数。胎儿肾上腺体积与胎儿体重呈正相关(r=0.943,P<0.001)。经胎儿体重校正的肾上腺体积随孕周无明显变化,其正常值范围为(0.691±0.122)ml/kg。结论本研究建立了各孕周的正常参考值范围和胎儿体重校正肾上腺体积的正常值范围,为临床评估胎儿肾上腺的大小与形态提供参考。 Purpose To explore the correlation between normal fetal adrenal gland volume(AGV)and gestational age(GA)and estimated fetal weight(EFW)in the middle and late trimester,and to establish reference ranges of it.Materials and Methods Three-dimensional ultrasound was used to obtain the images of adrenal gland of normal singleton pregnancies between 21 and 38+6weeks of gestation at the Second Xiangya Hospital of Central South University from March to December 2021,and the AGVs were obtained by VOCAL method.Various regression models were fitted to establish the reference ranges of AGV and to explore the relationship between AGV and EFW.Results A total of 590 fetuses of AGV were measured successfully,and the percentages of AGV at each GA were successfully established based on quadratic polynomial regression model(R^(2)=0.896,P<0.001).Fetal AGVs were positively correlated with EFW(r=0.943,P<0.001).The AGV corrected by EFW had no significant changes with GA,its reference range was(0.691±0.122)ml/kg.Conclusion The reference ranges of AGV and corrected by ETW and three-dimensional ultrasound via three-dimensional ultrasound were established,which provides reference for clinical evaluation of fetal adrenal gland size and morphology.
