摘要
目的:探讨NT-proBNP、Lac、cTnI联合检测与脓毒症心力衰竭预测的相关性。方法:选取医院重症医学科收治的82例脓毒症心力衰竭患者,所有患者均进行NT-proBNP(N末端B型利钠肽原)、Lac(动脉血乳酸)、cTnI(肌钙蛋白I)检测,并应用ROC曲线下面积(AUC)以对NT-proBNP、cTnI、Lac在脓毒症心力衰竭患者诊断中的敏感性、特异性进行统计,以对NT-proBNP、cTnI、Lac评估患者预后的价值进行分析。结果:不同心功能分级脓毒症心力衰竭患者NT-proBNP、Lac、cTnI水平比较,差异均有统计学意义(P<0.05),患者的心功能越差,NT-proBNP、Lac、cTnI水平越高。单独采用NT-proBNP检测的敏感性为75.61%,单独采用Lac检测的敏感性为64.63%,单独采用cTnI检测的敏感性为73.17%,脓毒症心力衰竭采用NT-proBNP、Lac、cTnI联合检测的敏感性为90.24%,与单项检测相比明显更高(P<0.05)。结论:采用NT-proBNP、Lac、cTnI联合检测方式能够有效地诊断早期脓毒症心力衰竭,从而为脓毒症心力衰竭的预防和早期诊治提供重要依据。
Objective:To analyze the correlation between the combined detection of NT proBNP,lac and cTnI and the prediction of sepsis heart failure.Methods:82 patients with sepsis and heart failure treated in the Department of critical medicine of hospital were selected as the research objects.NT proBNP(N-terminal B-type natriuretic peptide),Lac(arterial blood lactic acid)and cTnI(troponin I)were detected in all patients.The area under the ROC curve(AUC)was used to count the sensitivity and specificity of NT proBNP,cTnI and lac in the diagnosis of sepsis heart failure,and to analyze the prognostic value of NT pro BNP,c Tn I and lac.Results:There was significant difference in the levels of NT pro BNP,lac and c Tn I in patients with non concentric functional grade sepsis heart failure(P<0.05).The worse the cardiac function,the higher the levels of NT pro BNP,lac and c Tn I.The sensitivity of NT pro BNP alone,lac alone and cTnI alone was 75.61%,64.63%and 73.17%,respectively.The sensitivity of NT proBNP,lac and c TnI combined in sepsis and heart failure was 90.24%,significantly higher than that of single detection(P<0.05).Conclusion:The combined detection of NT proBNP,lac and cTnI can effectively diagnose early septic heart failure,so as to provide an important basis for the prevention and early diagnosis and treatment of septic heart failure.
