后半规管良性阵发性位置性眩晕复位后残余症状的影响因素

Influencing Factors of Residual Symptoms After Reduction of Posterior Semicircular Canal Benign Paroxysmal Positional Vertigo

目的探讨后半规管良性阵发性位置性眩晕(PC-BPPV)复位后残余症状的影响因素。方法选取2020年12月至2022年2月驻马店市第一人民医院收治的100例PC-BPPV患者。所有患者均接受改良式耳石复位法治疗,根据复位后残余症状发生情况将患者分为有残余症状组与无残余症状,使用多因素二元logistic回归分析PC-BPPV复位后残余症状的影响因素。结果100例PC-BPPV患者复位后有17例出现残余症状,发生率为17.00%,其中头晕5例,占比29.41%,不稳2例,占比11.76%,颈部不适4例,占比23.54%,合并2种残余症状5例,占比29.41%,合并3种残余症状1例,占比5.88%。多因素二元logistic回归分析显示病程不是PC-BPPV患者复位后发生残余症状的相关影响因素(P>0.05),年龄、糖尿病、高血压、半规管受损、复位次数≥3次、前庭症状指数(VSI)评分≥41分、眩晕残障量表(DHI)得分、焦虑抑郁量表(HADS)得分均为PC-BPPV患者复位后发生残余症状的相关影响因素(P<0.05)。结论年龄、糖尿病、高血压、半规管受损、复位次数≥3次、VSI评分≥41分、DHI得分、HADS得分均为PC-BPPV患者复位后发生残余症状的相关影响因素。

Objective To explore the influencing factors of residual symptoms after reduction of posterior semicircular canal benign paroxysmal positional vertigo(PC-BPPV).Methods A total of 100 patients with PC-BPPV admitted to the First People’s Hospital of Zhumadian from December 2020 to February 2022 were selected.All patients were treated with modified otolith reduction method.According to the occurrence of residual symptoms after reduction,the patients were divided into with residual symptoms group and without residual symptoms group according to the occurrence of residual symptoms after reduction.Multivariate binary logistic regression was used to analyze the influencing factors of residual symptoms after PC-BPPV reduction.Results After reduction,17 of 100 PC-BPPV patients had residual symptoms,with an incidence of 17.00%,including 5 cases of dizziness,accounting for 29.41%,2 cases of instability,accounting for 11.76%,4 cases of neck discomfort,accounting for 23.54%,5 cases of 2 residual symptoms,accounting for 29.41%,and 1 case of 3 residual symptoms,accounting for 5.88%.Multivariate binary logistic regression analysis showed that the course of disease was not the relevant influencing factor for the residual symptoms of PC-BPPV patients after reduction(P>0.05),but age,diabetes mellitus,hypertension,semicircular canal damage,reduction times≥3,vestibular symptom index(VSI)score≥41,dizziness handicap inventory(DHI)score,hospital anxiety and depression scale(HADS)score were all relevant influencing factors for the residual symptoms of PC-BPPV patients after reduction(P<0.05).Conclusion Age,diabetes mellitus,hypertension,semicircular canal damage,reduction times≥3,VSI score≥41,DHI score,HADS score are all the related factors for the residual symptoms of PC-BPPV patients after reduction.
