摘要
目的探讨远红外线治疗对血液透析患者动静脉内瘘(arteriovenous fistula,AVF)的影响。方法选取102例血液透析患者为研究对象,按照数字表法随机分为研究组与对照组,每组51例。对照组采取常规治疗,研究组在对照组基础上联合远红外线治疗。记录2组患者血肿吸收时间及透析血流量差异;比较治疗前及治疗3个月后2组患者疼痛程度[数字评分法(numerical rating scale,NRS)]评分和血管情况[血管内径、动脉阻力指数(resistance index,RI)];比较2组患者在治疗3个月内并发症(渗血、栓塞、感染)发生情况。结果研究组患者血肿吸收时间[(2.19±0.36)d]短于对照组[(3.75±0.92)d],且研究组透析血流量[(360.18±72.85)ml/min]大于对照组[(335.74±62.83)ml/min],差异均有统计学意义(P<0.05)。治疗3个月后,研究组患者疼痛程度较治疗前降低(P<0.05),且低于对照组(P<0.05);研究组血管内径较治疗前增大,动脉RI较治疗前减小(P<0.05),且均优于对照组(P<0.05)。治疗3个月内,研究组并发症总发生率(12.0%)较对照组(30.0%)低,差异有统计学意义(P<0.05)。结论远红外线治疗有利于缩短血液透析患者血肿吸收时间,提升透析血流量,改善其疼痛程度及血管情况,降低AVF相关并发症发生率。
Objective To investigate the effects of far‐infrared therapy on arteriovenous fistula(AVF)in the patients with hemodialysis.Methods A total of 102 patients with hemodialysis were selected as research subjects,and were divided into the study group and the control group according to the random number table method,each consisting 51 patients.The control group received conventional treatment,while the study group was treated with far‐infrared therapy on the basis of the control group.The differences in hematoma absorption time and dialysis blood flow were recorded for the the patients of the 2 groups.The pain level by using numerical rating scale(NRS)and vascular conditions including vascular diameter and arterial resistance index(RI)before treatment and 3 months after treatment were compared between the two groups.The occurrence of complications(exudation,embolism and infection)was analyzed in the two groups within 3 months of treatment.Results The hematoma absorption time of the study group(2.19±0.36)days was shorter than that of control group(3.75±0.92)days,with statistical significance(P<0.05),and the dialysis blood flow of the study group(360.18±72.85)ml/min was greater than that of control group(335.74±62.83)ml/min,also with statistical significance(P<0.05).After 3 months of treatment,the NRS scores of the study group was lower than that before treatment(P<0.05),and was lower than that of the control group(P<0.05).The vascular diameter of the study group were larger,and arterial RI of the study group were smaller than those before treatment(P<0.05),and were also better than those of the control group(P<0.05).3 months after treatment,the total rate of complications in the study group(12.0%)was lower than that in control group(30.0%),with statistical significance(P<0.05).Conclusion Far‐infrared therapy could help to shorten the hematoma absorption time,increase the blood flow of hemodialysis,improve the pain level and vascular conditions,and more importantly reduce complication rate of AVF‐related patients with hemodialysis.
