摘要
目的:基于磁共振胰胆管成像(MRCP)技术分析胆总管几何结构参数与胆囊结石发生的相关性。方法:选取2019年1月—2021年6月于中国人民解放军北部战区总医院行MRCP检查的180例患者作为研究对象,根据有无胆囊结石分为胆囊结石组与无胆囊结石组,依据冠状位3D-MRCP图像,比较两组胆总管的转角、长度、最大离散距离、最大直径、转角处直径。结果:两组胆总管转角、最大离散距离、最大直径、转角处直径比较,差异无统计学意义(P>0.05);胆囊结石组胆总管长度长于无胆囊结石组,差异有统计学意义(P<0.001)。胆总管长度诊断胆囊结石的受试者操作特征曲线下面积为0.672,最佳截断值为59.35mm。结论:胆囊结石的发生与胆总管几何结构有一定相关性,当胆总管长度>59.35mm时,临床医生应高度重视,采取针对性措施预防胆囊结石的发生。
Objective:To analyze the correlation between geometric structure parameters of common bile duct and occurrence of gallstones based on magnetic resonance cholangiopancreatography(MRCP).Methods:A total of 180 patients who underwent MRCP examination in General Hospital of the Northern Theater Command of the Chinese People's Liberation Army from January 2019 to June 2021 were selected as the study subjects.According to the presence/absence of gallstones,they were divided into gallstones group and no-gallstones group.Based on the coronary 3D-MRCP image,the common bile duct angle,length,maximum discrete distance,maximum diameter and diameter at the angle were compared between two groups.Results:There was no statistical difference in the common bile duct angle,maximum discrete distance,maximum diameter,and diameter at the angle between the two groups(P>0.05).The length of the common bile duct in the gallstones group was longer than that in the no-gallstones group,and the difference was statistically significant(P<0.001).The area under the receiver operating characteristic curve of common bile duct length for the diagnosis of gallstones was 0.672,and the optimal cut-off value was 59.35 mm.Conclusion:The occurrence of gallstones is related to geometric structure of the common bile duct,and clinicians should pay high attention and take the targeted measures to prevent the occurrence of gallstones when the length of the common bile duct is>59.35 mm.
