摘要
目的观察大柴胡汤对结石性胆囊炎豚鼠血清炎症因子水平和胆囊动力的影响,探讨大柴胡汤治疗结石性胆囊炎的作用机制。方法将40只雌性豚鼠随机分为正常组、模型组、消炎利胆片组、大柴胡汤组,每组10只。模型组、消炎利胆片组、大柴胡汤组连续喂以高胆固醇致石饲料4周建立结石性胆囊炎模型。造模成功后,大柴胡汤组给予大柴胡汤颗粒剂灌胃,消炎利胆片组给予消炎利胆片灌胃,正常组和模型组给予等量生理盐水灌胃,均连续灌胃4周。灌胃结束后,腹主动脉取血测定血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、胆囊收缩素(CCK)、小窝蛋白-3(Cav-3)水平,测量胆汁量,取胆囊检测胆囊肌条收缩活性。结果与正常组比较,模型组豚鼠血清TNF-α、IL-1β、Cav-3水平均明显升高(P均<0.05),血清CCK水平明显降低(P<0.05),胆汁量明显减少(P<0.05),胆囊收缩频率明显减慢(P<0.05),胆囊收缩振幅无明显变化(P>0.05)。与模型组比较,消炎利胆片组和大柴胡汤组豚鼠血清TNF-α、IL-1β、Cav-3水平均明显降低(P均<0.05),血清CCK水平均明显升高(P均<0.05),胆汁量均明显增加(P均<0.05),胆囊收缩频率均明显变快(P均<0.05),胆囊收缩振幅均无明显变化(P均>0.05)。与消炎利胆片组比较,大柴胡汤组豚鼠胆汁量更多,血清TNF-α、IL-1β、CCK水平更高,血清Cav-3水平更低,胆囊肌条收缩频率更快,差异均有统计学意义(P均<0.05)。结论大柴胡汤可以明显减轻结石性胆囊炎豚鼠的炎症反应,增加胆汁量,促进胆囊运动,其作用机制可能与下调血清TNF-α、IL-1β水平发挥抗炎作用,上调血清CCK水平并下调血清Cav-3水平发挥利胆作用相关。
Objective It is to observe the effect of Major Bupleurum Deccotion on serum inflammatory factors and gallbladder motility in guinea pigs with calculous cholecystitis,and to explore the mechanism of this decoction in the treatment of calculous cholecystitis.Methods Forty female guinea pigs were randomly divided into normal group,model group,tablet for diminishing inflammation and disinhibiting gallbladder(TDIDG)group and Major Bupleurum Deccotion(MBD)group,with 10 in each group.The guinea pigs in the model group,TDIDG group and MBD group were continuously fed with high cholesterol stone-inducing diet for 4 weeks to establish the models of calculous cholecystitis.After successful modeling,the MBD group was given MBD granules by gavage,the TDIDG group was given TDIDG by gavage,the normal group and model group were given the same amount of normal saline by gavage,all continuously treated for 4 weeks.At the end of gavage,the blood samples from abdominal aorta of the guinea pigs were taken to measure the levels of serum tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),cholecystokinin(CCK),capsulin-3(Cav-3),the bile volume and contractile activity of gallbladder muscle strips were also detected.Results Compared with the normal group,the levels of serum TNF-α,IL-1β,and Cav-3 were significantly increased(all P<0.05),the level of serum CCK level was significantly decreased(P<0.05),the bile volume was significantly reduced(P<0.05),the contraction frequency of gallbladder was significantly slowed in guinea pigs of the model group(P<0.05),and there was no significant difference in the amplitude of gallbladder contraction between the two group(P>0.05).Compared with the model group,the levels of serum TNF-α,IL-1β,and Cav-3 were significantly decreased(all P<0.05),the level of serum CCK level was significantly increased(P<0.05),the bile volume was significantly increased(P<0.05),the contraction frequency of gallbladder was significantly accelerated in guinea pigs of the TDIDG group and MBD group(P<0.05),and there was no significant difference in the amplitude of gallbladder contraction among the three groups(P>0.05).Compared with the TDIDG group,the bile volume was more,the levels of serum TNF-α,IL-1β,CCK were higher,the level of serum Cav-3 was lower,and the contraction frequency of of gallbladder muscle strips was faster in guinea pigs of the MBD group,and the differences were all statistically significant(all P<0.05).Conclusion Major Bupleurum Deccotion can effectively alleviate the inflammatory response of guinea pigs with calculous cholecystitis,increase bile volume,promote gallbladder movement and enhance gallbladder dynamics,so as to treat calculous cholecystitis.The mechanism of action may be related to its anti-inflammatory effect via down-regulating the levels of serum TNF-α and IL-1β,and related to its disinhibiting gallbladder effect via up-regulating the level of serum CCK and down-regulating the level of serum CAV-3.
