摘要
目的探讨慢性牙周炎患者外周血辅助性T细胞1(Th1)、辅助性T细胞2(Th2)、辅助性T细胞17(Th17)及牙周组织白细胞介素-17(IL-17)、干扰素-γ(IFN-γ)表达与感染机制的相关性。方法选取2019年10月至2021年1月本院收治的慢性牙周炎患者110例作为观察组,按照慢性牙周炎的严重程度分为重度组(n=49)与轻中度组(n=61),选取同期60名牙周健康者作为健康组,比较3组外周血Th1、Th2、Th17水平,分析牙周组织HE染色检测结果,比较观察组与健康组IL-17、IFN-γ细胞H-score评分。结果重度组、轻中度组外周血Th1、Th2、Th17水平均高于健康组,且重度组Th1、Th17水平高于轻中度组,差异有统计学意义(P<0.05)。轻度牙周炎患者淋巴细胞数目少于中、重度牙周炎患者,且中度牙周炎患者少于重度牙周炎患者,差异有统计学意义(P<0.05)。观察组牙周组织IL-17、IFN-γ细胞的H-score评分均高于健康组,差异有统计学意义(P<0.05)。结论慢性牙周炎患者外周血Th1、Th2、Th17及牙周组织IL-17、IFN-γ表达升高,与感染相关。
Objective To investigate the correlation between the expression of peripheral blood T helper cell 1(Th1),T helper cell 2(Th2),T helper cell 17(Th17)and gingival tissue interleukin-17(IL-17)and interferon-γ(IFN-γ)and the infection mechanism in patients with chronic periodontitis.Methods A total of 110 patients with chronic periodontitis admitted to our hospital from October 2019 to January 2021 were selected as the observation group,and they were divided into the severe group(n=49)and the mild to moderate group(n=61)according to the severity of chronic periodontitis,60 periodontal healthy subjects at the same time were selected as the healthy group,the levels of Th1,Th2 and Th17 in peripheral blood were compared among the three groups,the results of gingival tissue HE staining were analyzed,and the H-score scores of IL-17 and IFN-γcells were compared between the observation group and the healthy group.Results The levels of Th1,Th2 and Th17 in peripheral blood in the severe group and the mild to moderate group were higher than those in the healthy group,and the levels of Th1 and Th17 in the severe group were higher than those in the mild to moderate group,the differences were statistically significant(P<0.05).The number of lymphocytes in patients with mild periodontitis was less than that in patients with moderate and severe periodontitis,and the number of lymphocytes in patients with moderate periodontitis was less than that in patients with severe periodontitis,the difference was statistically significant(P<0.05).The H-score scores of IL-17 and IFN-γcells in gingival tissues in the observation group were higher than those in the healthy group,and the differences were statistically significant(P<0.05).Conclusion The expressions of Th1,Th2,Th17 in peripheral blood and IL-17 and IFN-γin gingival tissue of patients with chronic periodontitis are increased,which is related to infection.
