二至地黄汤加减治疗小儿过敏性紫癜性肾炎的有效性及临床价值

Effectiveness and Clinical Value of Modified Erzhi Dihuang Decoction in the Treatment of Pediatric Allergic Purpura Nephritis

目的分析小儿过敏性紫癜性肾炎应用二至地黄汤加减治疗的临床有效性和价值。方法以2021年1月至2022年12月为研究时间区间,从郓城县中医医院甄选小儿过敏性紫癜性肾炎患儿进行研究,共计30例,患儿全部就位之后,采用随机数表法,将患者均分为两个组别,对照组(15例)和观察组(15例)。对照组实施常规西药治疗,观察组于常规西药治疗基础上实施二至地黄汤加减治疗。对比观察组和对照组的治疗效果、炎症因子水平变化情况、中医症候积分变化情况以及不良反应发生情况。结果较对照组患儿,观察组治疗总有效率较高(P<0.05),白细胞介素-4(interleukin-4,IL-4)、干扰素-γ(interferon-γ,IFN-γ)治疗前,对照组和观察组比较相差不大(P>0.05),经治疗后,IL-4与IFN-γ水平对照组和观察组比较,观察组均优于对照组(P<0.05)。中医症候积分治疗前,对照组和观察组比较相差不大(P>0.05),经治疗后,中医症候积分降低,且观察组患者的中医症候积分更低(P<0.05)。观察组患者丙氨酸氨基转移酶升高发生率、胃肠反应以及白细胞减少发生率低于对照组(P<0.05)。结论小儿过敏性紫癜性肾炎应用二至地黄汤加减治疗的作用效果显著,可改善患儿血清免疫指标、炎症因子水平以及中医症候积分,利于患儿病情康复,且安全性较高。

Objective To analyze the clinical efficacy and value of modified Erzhi Dihuang decoction in the treatment of pediatric allergic purpura nephritis.Methods From January 2021 to December 2022 as the time interval of the study,from Yuncheng County Hospital of Traditional Chinese Medicine selected children with paediatric allergic purpura nephritis for the study,a total of 30 cases,after all the children were in place,using the random number table method,the patients were divided into two groups control group(15 cases)and observation group(15 cases).The control group was treated with conventional western medicine,while the observation group was treated with Erzhi Di Huang Tang on the basis of conventional western medicine.The therapeutic effects,changes in the levels of inflammatory factors,changes in the TCM symptom scores,and the occurrence of adverse reactions were compared between the observation group and the control group.Results Compared with the children in the control group,the total effective rate of treatment in the observation group was higher(P<0.05),and there was no significant difference between the control group and the observation group in the levels of interleukin-4(IL-4,IL-4)and interferon-γ(interferon-γ,IFN-γ)before the treatment(P>0.05),and the differences between the control group and the observation group were not significant(P>0.05).observation group were better than the control group(P<0.05).Before treatment,the difference between the control group and the observation group was not significant(P>0.05),and after treatment,the TCM symptom score was reduced,and the patients in the observation group had a lower TCM symptom score(P<0.05).The incidence of alanine aminotransferase elevation,gastrointestinal reaction and leukopenia in the observation group was lower than that in the control group(P<0.05).Conclusions The application of modified Erzhi Dihuang decoction in the treatment of children with allergic purpura nephritis has a significant effect.It can improve the serum immune indicators,inflammatory factor levels,and traditional Chinese medicine syndrome scores of children,which is beneficial for their recovery and has high safety.
