131I治疗后甲状腺危象并糖尿病酮症酸中毒致多器官衰竭1例

Multiple organ failure caused by thyroid storm and diabetes ketoacidosis following radioactive iodine-131 treatment:a case report

甲状腺危象(TS)和糖尿病酮症酸中毒(DKA)都是内分泌急症,任意一种急症都会诱发另一种急症。131I治疗后出现TS并DKA的病例报告较少见,但死亡率较高,因此有早期识别和积极治疗对于防止进一步失代偿至关重要。本研究报道1例131I治疗后甲状腺危象并糖尿病酮症酸中毒致多器官衰竭的诊治过程并文献复习,希望为TS合并DKA患者的临床治疗提供更多的资料及经验。

Thyroid storm(TS)and diabetic ketoacidosis(DKA)are both endocrine emergencies.Either condition can induce the other.Radioiodine induced thyroid storm complicated with diabetic ketoacidosis is rare,however,the mortality is high.Thus,early recognition and aggressive treatment are crucial in preventing further decompensation.We reported a case of multiple organ failure caused by radioiodine induced thyroid storm complicated with diabetic ketoacidosis,with detailed description of his diagnosis and treatment process and carried out a literature review,hoping to inform clinical treatment of TS patients with DKA.
