摘要
目的研究剪切波弹性成像(SWE)联合彩色多普勒超声对后方回声衰减型乳腺肿物良恶性的鉴别诊断价值。方法选取后方回声衰减型乳腺肿物患者100名,随机均分为对照组(彩色多普勒超声)和联合组(SWE联合彩色多普勒超声),按病理结果分为良性组和恶性组。比较各组超声诊断结果和SWE参数。根据病理结果评价SWE联合彩色多普勒超声的诊断效力。结果与单纯SWE诊断及单纯超声诊断相比,联合组SWE+超声诊断的假阳性率最低;恶性组SWE弹性模量平均值(E_(mean))、最大值(E_(max))明显高于良性组。ROC曲线分析发现,SWE和彩色多普勒超声联合检测对后方回声衰减型乳腺肿物良恶性的鉴别诊断价值高于单独检测。结论SWE联合彩色多普勒超声检测可更精准地评估后方回声衰减型乳腺肿物的良恶性,但不排除发生假阳性结果的可能。
Aim To study the value of shear wave elastography(SWE)combined with color Doppler ultrasound in differential diagnosis of benign and malignant breast tumors with backward echo attenuation.Methods 100 patients with breast masses were randomly divided into control group(color Doppler ultrasound)and combined group(SWE combined with color Doppler ultrasound).According to the pathological results,they were divided into benign group and malignant group.The ultrasonographic diagnosis results and SWE parameters of each group were compared.The diagnostic efficacy of SWE combined with color Doppler ultrasound was evaluated according to the pathological results.Results Compared with SWE diagnosis alone and ultrasound diagnosis alone,the false positive rate of the combined group was lower.The mean value(E_(mean))and maximum value(E_(max))of SWE in the malignant group were significantly higher than those in the benign group.The ROC curve analysis showed that the combined detection of SWE and color Doppler ultrasound was more valuable than the single detection in the differential diagnosis of benign and malignant breast tumors with backward echo attenuation.Conclusion SWE combined with color Doppler ultrasonography can more accurately evaluate the benign and malignant tumors of the posterior echo attenuation type,but it does not rule out the possibility of false positive results.
