甲状腺微小乳头状癌颈部淋巴结转移的原发灶超声特征及诊断价值研究

摘要 目的 探究甲状腺微小乳头状癌(PTMC)颈部淋巴结转移的原发灶超声特征及诊断价值。方法选取2019年1月—2023年3月南京市江宁医院收治的133例PTMC患者为研究对象,根据疾病类型分为对照组(无颈部淋巴结转移,n=74)与观察组(存在颈部淋巴结转移,n=59),均行超声诊断,对比两组超声特征。结果 超声检查诊断阳性患者50例、阴性65例;超声检查敏感度86.21%、特异度87.84%、准确度87.12%;病理学检查诊断一致性为最强,而超声检查为高度;两组结节最大直径、与甲状腺被膜接触、结节形态、纵横径比、钙化对比差异具有统计学意义(P<0.05)。结论 采取超声检查诊断PTMC疾病,效果良好,可通过两组结节最大直径、与甲状腺被膜接触、结节形态、纵横径比、钙化等指标判断是否存在淋巴结转移,临床意义显著。 Objective To investigate ultrasound characteristics and diagnostic value of primary lesion of cervical lymph node metastasis of papillary thyroid microcarcinoma(PTMC).Methods The paper chose 133 cases PTMC patients admitted to Nanjing Jiangning Hospital from January 2019 to March 2023 as study subjects,and divided them into control group(without cervical lymph node metastasis,n=74)and observation group(with cervical lymph node metastasis,n=59)based on type of disease,treated with ultrasound diagnosis,ultrasound characteristics between two groups was compared.Results Ultrasound examination diagnosis showed 50 positive patients and 65 negative patients,with sensitivity of 86.21%,specificity of 87.84%,accuracy of 87.12%.Pathological examination showed the strongest diagnostic consistency,while ultrasound examination was highly consistent,with statistically significant difference in maximum diameter,thyroid capsule contact,nodule morphology,aspect ratio,and calcification between two groups(P<0.05).Conclusion Ultrasound can achieve good diagnostic effect on PTMC disease.Clinical diagnosis of lymph node metastasis can be made based on indicators of maximum diameter of nodules,thyroid capsule contact,nodule morphology,aspect ratio,and calcification,with significant clinical significance.
