Rockall评分理念下的分级护理在上消化道出血患者生长抑素治疗中的干预价值研究

Study of the Intervention Value of Graded Care under the Rockall Score Concept in the Treatment of Growth Inhibitors in Patients with Upper Gastrointestinal Bleeding

目的:探讨在上消化道出血患者生长抑素治疗中应用Rockall评分理念下分级护理的干预价值。方法:选取2018年7月—2020年8月郑州大学第一附属医院消化内二科收治的76例自愿参与调查的上消化道出血患者作为研究对象,按照基础资料具有匹配性的原则分为研究组及对照组,每组各38例。对照组患者给予常规护理,研究组实施Rockall评分理念下的分级护理。比较两组患者出血情况、生活质量评分及Rockall评分。结果:干预后,研究组患者比对照组出血时间更短,出血量更低,再次出血次数更少,差异有统计学意义(t=29.888、23.602、16.259,P<0.05)。干预后,研究组患者生活质量各维度评分均高于对照组,差异有统计学意义(t=10.031、10.046、9.785、9.278、9.101、9.682,P<0.05)。干预后,研究组患者Rockall评分明显低于对照组,差异有统计学意义(t=3.364,P<0.05)。结论:在上消化道出血患者生长抑素治疗中应用Rockall评分理念下分级护理,可降低患者出血量,提高其生活质量。

Objective:To investigate the value of interventions applying the Rockall score concept of graded care in the treatment of patients with upper gastrointestinal bleeding with growth inhibitors.Methods:76 patients with upper gastrointestinal bleeding admitted to the hospital from July 2018 to August 2020 who voluntarily participated in the survey were selected as study subjects and divided into study group and control group according to the principle of having matching basic data,with 38 cases in each group.The control group was given conventional care and the study group implemented graded care under the Rockall score concept.The patients’bleeding,quality of life scores and Rockall scores were compared between the two groups.Results:After the intervention,the study group had shorter bleeding time,lower bleeding volume and fewer rebleeds than the control group,with statistically significant differences(t=29.888,23.602,16.259,P<0.05).After the intervention,the study group scored higher than the control group in all dimensions of quality of life,with statistically significant differences(t=10.031,10.046,9.785,9.278,9.101,9.682,P<0.05).After the intervention,the Rockall score was significantly lower in the study group than in the control group,with a statistically significant difference(t=3.364,P<0.05).Conclusion:Application of graded care under the Rockall score concept in the treatment of patients with upper gastrointestinal bleeding reduces bleeding and improves their quality of life.
