摘要
目的:探讨量化踝泵运动联合气压治疗仪预防前列腺癌根治术后下肢静脉血流动力学及PTS的影响。方法:选取2019年1月至2022年7月行前列腺癌根治手术患者60例,随机分为两组,每组30例。观察组在对照组基础上实施量化踝泵运动联合双下肢气压治疗仪干预;对照组实施常规护理。比较两组患者手术前、术后3 d下肢股静脉内血流平均速度和峰速度、凝血四项及D-二聚体(D-D)变化。结果:两组患者术后3 d下肢股静脉内血流平均速度和峰速度相比较,观察组低于对照组,差异有统计学意义(P<0.05)。两组患者术前D-二聚体、凝血酶原时间(PT)、活化部分凝血酶时间APTT、血浆纤维蛋白原(FBG)、凝血酶(TT)时间水平差异无统计学意义(P>0.05)。术后3 d两组患者PT、APTT较手术前延长,而空腹血糖(FBG)、D-D及TT下降,两组比较差异有统计学意义(P<0.05)。结论:前列腺癌根治术后联合应用量化踝泵运动及气压治疗仪治疗,可促进血液循环,预防下肢静脉血栓的形成。
Objective:To evaluate the effects of ankle pump exercise combined with pneumatic pump on lower limb venous blood flow dynamics and post-thrombotic syndrome(PTS)after radical prostatic cancer surgery.Methods:A total of 60 patients who underwent radical prostatectomy from January 2019 to July 2022 were selected and randomly divided into two groups,30 cases in each group.On the basis of the control group,the observation group was given quantitative ankle pump exercise combined with double lower extremity air pressure pump intervention.The control group was given routine nursing care,and the average and peak blood flow velocity in the lower extremity femoral vein before operation and 3 days after operation,and the changes of coagulation four items and D-dimer(D-D)were compared between the two groups.Results:There were no significant differences in preoperative(D-D),prothrombin time,activated partial thrombin time,plasma fibrinogen and thrombin time between 2 groups(P>0.05).PT and APTT in 2 groups were longer than before 3 days after operation;FBG,D-D and TT decreased,and the difference between the two groups was statistically significant(P<0.05).Conclusion:Combined application of quantitative ankle pump exercise and pneumatic therapy pump therapy after radical prostatectomy can promote blood circulation and prevent the formation of lower extremity venous thrombosis.
