静脉药物配置中心不合理用药的分析

Analysis of unreasonable drug use in Pharmacy intravenous admixture service

目的对静脉用药不合理医嘱进行分析,为提高临床静脉用药的合理性提供依据,确保用药的安全性。方法选取静脉药物配置中心(PIVAS)2022年1~12月审方药师干预的不合理医嘱,并进行分析。结果2022年1~12月PIVAS不合理医嘱共计1899例,涉及溶媒种类和用量、配伍禁忌、药物用法用量、给药途径、全肠外营养液(TPN)组分等问题。结论PIVAS审方药师对不合理用药的医嘱能够有效干预,确保患者的用药安全,达到有效治疗的目的。

Objective To analyze the unreasonable medical orders of intravenous medication,in order to provide a basis for improving the rationality of clinical intravenous medication and ensuring the safety of the medication.Methods Select and analyze unreasonable medical orders in Pharmacy intravenous admixture service intervened by prescription-checking pharmacists from January to December 2022.Results From January to December 2022,there were 1899 cases of unreasonable medical orders in PIVAS,involving the types and dosage of solvents,incompatibilities,usage and dosage of drugs,route of administration,composition of TPN,etc.Conclusion Prescription-checking pharmacists of PIVAS can effectively intervene in unreasonable medication orders,ensure patient medication safety,and achieve the goal of effective treatment.
