摘要
目的探究老年患者身高缩短距离与头臂干扭曲的相关性。方法选择2020年10月至2021年10月嘉善县第一人民医院收治的可疑冠心病患者222例,均采用右侧桡动脉路径行冠状动脉造影。当亲水导丝在头臂干中的弯曲角度超过180°时,判定为头臂干扭曲。根据造影结果分为头臂干扭曲患者(观察组)86例和无头臂干扭曲患者(对照组)136例。记录成年身高与入院时身高,测定骨密度,采用Spearman秩相关分析身高缩短距离与头臂干扭曲的相关性,采用ROC曲线评估身高缩短距离对头臂干扭曲的诊断效能。结果观察组女性比例高于对照组,骨量减少、骨质疏松比例高于对照组,身高缩短距离大于对照组,差异均有统计学意义(均P<0.05)。Spearman秩相关分析显示身高缩短距离与头臂干扭曲呈正相关(r_(s)=0.662,P=0.001)。ROC曲线显示,身高缩短距离是预测头臂干扭曲的可靠指标,AUC为0.89,截断值为5.1cm。结论身高缩短是头臂干扭曲的重要影响因素。
Objective To explore the correlation between the degree of body height reduction and brachiocephalic artery(BA)tortuosity in elderly patients.Methods A total of 222 patients with suspected coronary heart disease admitted to the First People's Hospital of Jiashan from October 2020 to October 2021 were enrolled.They underwent a diagnostic cardiac catheterization procedure via right radial approach.BA tortuosity was determined when the bending angle of the hydrophilic guide wire in BA exceeds 180°.According to the imaging results,there were 86 patients with BA tortuosity(observation group)and 136 patients without BA tortuosity(control group).Patient's adult height and admission height were recorded.Bone mineral density was measured.Spearman rank correlation analysis was used to analyze the correlation between height reduction and BA tortuosity.ROC curve was used to evaluate the diagnostic efficacy of height reduction on the BA tortuosity.Results The proportion of women in the observation group was significantly higher than that in the control group(P<0.05).The degree of height reduction and the prevalence of osteoporosis in the observation group were significantly higher than those in the control group(all P<0.05).Spearman rank correlation analysis showed that height reduction was positively correlated to BA tortuosity(r_(s)=0.662,P=0.001).The ROC curve indicated that height reduction was a sensitive predictor for BA tortuosity(P<0.05),with an AUC of 0.89 and a cut-off value of 5.1 cm.Conclusion Body height reduction is an important factor affecting BA tortuosity.
