深圳市福永医院社康首诊伤害流行病学调查及处理情况分析

Epidemiological Investigation and Analysis of Treatment of First Diagnosis Injury of Shekang in Fuyong Hospital,Shenzhen

目的:分析深圳市福永医院社区卫生服务中心(简称“社康”)首诊伤害流行病学特征及处理情况,为伤害防范干预提供参考依据。方法:回顾性分析2020年1—12月于集兴围社康、和平社康就诊的首诊伤害病例2041例的临床资料,使用《中国伤害监测报告卡》进行数据收集,分析患者一般资料、首诊伤害情况、处理情况。结果:2041例首诊伤害的病例中男女比例1.92∶1;年龄以30~44岁人群首诊伤害的占比最高。各个年龄段伤害类型分布情况比较,差异有统计学意义(P<0.05)。各个年龄段损伤部位分布情况比较,差异有统计学意义(P<0.05)。2041例首诊伤害者中,采取自行处理方式748例(36.65%),1293例(63.35%)前往医院就诊处理;转诊人数425例,转诊比例为20.82%;因止血效果不佳转诊70例(3.43%),因持续高热不退转诊58例(2.84%),因大面积烧烫伤转诊142例(6.96%),因致伤部位伤口大转诊60例(2.94%),因多部位损伤及合并疾病转诊95例(4.65%);其中转诊方向为三级医院185例(9.06%),二级医院240例(11.76%)。结论:深圳市福永医院社康首诊伤害病例中男性比例较高,损伤原因儿童以烧烫伤为主,中青年以锐器伤为主,老年以跌倒为主,伤害多选择进入医院就诊,转诊情况一般,需根据首诊患者基本情况予以诊治,病情严重者积极转诊确保救治效果。

Objective:To analyze the epidemiological characteristics and treatment of injuries at first diagnosis in Community Health Service Center of Shenzhen Fuyong Hospital("Shekang"for short),and provide reference for injury prevention and inter-vention.Methods:The clinical data of two thousand and forty-one first-diagnosed injury cases in Jixingwai and Heping Shikang from January 2020 to December 2020 were retrospectively analyzed,and the data were collected using the"China Injury Monitor-ing Report Card".The general data,first diagnosis injury and treatment of patients were analyzed.Results:The male to female ra-tio of 2,041 first diagnosed injuries was 1.92∶1;The proportion of first diagnosed injury was the highest in people aged 30-44 years.There were statistical significance in the distribution of injury types in each age group(P<0.05).There was statistical signifi-cance in the distribution of injury sites in each age group(P<0.05);Among 2,041 patients,748(36.65%)took self-treatment and 1,293(63.35%)went to hospital for treatment;The number of referrals was 425(20.82%);70 cases(3.43%)were referred due to poor hemostatic effect,58 cases(2.84%)were referred due to persistent high fever,142 cases(6.96%)were referred due to exten-sive burns and scalds,60 cases(2.94%)were referred due to large wounds at the injury site,and 95 cases(4.65%)were referred due to multi-site injuries and complicated diseases;Among them,185 cases(9.06%)were referred to tertiary hospitals and 240 cases(11.76%)were referred to secondary hospitals.Conclusion:In Shenzhen Fuyong Hospital Shekang,the proportion of males in the first diagnosis of injuries was high.The injuries were mainly caused by burns in children,sharp instrument injuries in middleaged and young people,and falls in the elderly.Most of the injured choose to go to the hospital for treatment,the referral situation is general,and it is necessary to diagnose and treat according to the basic situation of the first patient,and the serious patients are ac-tively referred to ensure the treatment effect.
