补肾化痰法治疗多囊卵巢综合征胰岛素抵抗研究进展

Research Progress on Treatment of Polycystic Ovary Syndrome with Insulin Resistance by Tonifying Kidney and Removing Phlegm

多囊卵巢综合征(polycystic ovarian syndrome,PCOS)是育龄期女性常见的生殖内分泌及代谢紊乱性疾病。胰岛素抵抗(insulin resisitance,IR)是PCOS发生发展的重要病理因素,且50%及以上的PCOS患者伴有IR。中医认为肾虚痰湿是多囊卵巢综合征胰岛素抵抗(PCOS-IR)的主要病机之一,西医认为PCOS-IR的发生机制可能与胰岛素信号通路异常、炎症反应、氧化应激和肠道菌群失调有关。补肾化痰法治疗PCOS-IR能够起到改善糖脂代谢、调节性激素、抗炎抗氧化的作用。文章就PCOS-IR的中医病机、西医发生机制及补肾化痰法在PCOS-IR中的临床应用进行综述,以期为补肾化痰法应用于临床PCOS-IR的诊疗提供依据。

Polycystic ovary syndrome(PCOS)is a common reproductive endocrine and metabolic disorders in women of childbearing age.Insulin resistance(IR)is an important pathological factor for the occurrence and development of PCOS,and 1/2 or more PCOS patients are accompanied by IR.According to traditional Chinese medicine,kidney deficiency and phlegm-dampness is one of the main pathogenesis of PCOS with insulin resistance(PCOS-IR),while Western medicine believes that the pathogenesis of PCOS-IR may be related to abnormal insulin signaling pathway,inflammatory response,oxidative stress and intestinal flora.The treatment of PCOS-IR by tonifying kidney and eliminating phlegm can improve glucose and lipid metabolism,regulate sex hormones,and fight inflammation and oxidation.This article reviewed the pathogenesis of PCOS-IR in traditional Chinese medicine,the pathogenesis of PCOS-IR in Western medicine and the clinical application of tonifying kidney and removing phlegm in PCOS-IR,in order to provide evidence for the application of tonifying kidney and removing phlegm in clinical treatment of PCOS-IR.
