剪切波弹性成像联合超微血管三维立体成像在乳腺良恶性结节鉴别诊断中的应用价值

摘要 目的:探讨剪切波弹性成像(SWE)联合超微血管三维立体成像(Smart 3D SMI)鉴别乳腺结节良恶性的价值。方法:选取行乳腺超声检查108例病人(118个结节)作为研究对象,所有病人均进行二维超声检查、SWE检查及Smart 3D SMI检查,并以病理结果作为金标准,比较SWE、Smart 3D SMI及两者联合检查的诊断效能。结果:SWE、Smart 3D SMI单独检测与病理结果的一致性均一般(P<0.01),SWE联合Smart 3D SMI与病理结果的一致性较好(P<0.01)。SWE联合Smart 3D SMI检查诊断乳腺肿瘤性质的敏感度、特异度、准确率均较SWE、Smart 3D SMI单独诊断高,差异有统计学意义(P<0.05~P<0.01)。结论:SWE联合Smart 3D SMI在乳腺结节良恶性鉴别诊断中具有较好的应用价值。 Objective:To explore the value of shear wave elastography(SWE)combined with smart three-dimensional superb microvascular imaging(Smart 3D SMI)in distinguishing benign and malignant breast nodules.Methods:A total of 108 patients(118 nodules)who underwent breast ultrasound examination were selected as the study subjects.All patients underwent two-dimensional ultrasound examination,SWE examination,and Smart 3D SMI examination,and the pathological results were used as the gold standard to compare the diagnostic efficacy of SWE,Smart 3D SMI,and their combination examination.Results:The consistency between SWE and Smart 3D SMI detection alone and pathological results was average(P<0.01)The consistency between SWE combined with Smart 3D SMI and pathological results was good(P<0.01).The sensitivity,specificity,and accuracy of SWE combined with Smart 3D SMI in diagnosing the nature of breast tumors were higher than those of SWE and Smart 3D SMI alone,and the difference was statistically significant(P<0.05 to P<0.01).Conclusions:The combination of SWE and Smart 3D SMI has good application value in the differential diagnosis of benign and malignant breast nodules.
