CEA、CA724、CA19-9、CA125联合检测在胃癌患者中的运用价值

Application Value of CEA,CA724,CA19-9 and CA125 Combined Detection in Patients with Gastric Cancer

目的观察血清癌胚抗原(CEA)、糖类抗原724(CA724)、糖类抗原19-9(CA19-9)、糖类抗原125(CA125)联合检测对胃癌患者的临床价值。方法选取我院2020年5月至2023年5月收治的176例胃癌患者作为恶性组,按照2∶1配对原则选取同期、同年段88名健康对照作为对照组。比较两组血清CEA、CA724、CA19-9、CA125水平,分析胃癌患者血清各指标与临床病理特征的关系,ROC曲线评估血清各指标联合对胃癌的诊断价值。结果恶性组血清CEA、CA724、CA19-9、CA125水平高于对照组(P<0.05);胃癌患者分化程度、TNM分期、淋巴结转移、远处转移血清CEA、CA724、CA19-9、CA125水平比较,差异有统计学意义(P<0.05);血清CEA、CA724、CA19-9、CA125与分化程度、TNM分期、淋巴结转移、远处转移呈正相关(P<0.05);血清CEA、CA724、CA19-9、CA125联合诊断胃癌的AUC大于各指标单独诊断(P<0.05)。结论胃癌患者血清CEA、CA724、CA19-9、CA125表达较高,与患者分化程度、TNM分期、淋巴结转移、远处转移有关,且联合检测胃癌具有较高参考价值。

Objective To observe the clinical value of serum carcinoembryonic antigen(CEA),carbohydrate antigen(CA)724,CA19-9,CA125 combined detection in patients with gastric cancer.Methods 176 patients with gastric cancer admitted to our hospital from May 2020 to May 2023 were selected as the malignant group,and 88 healthy controls were selected as the control group according to the 2:1 pairing principle.Serum CEA,CA724,CA19-9 and CA125 levels were compared between the two groups,and the relationship between serum indexes and clinicopathological characteristics of patients with gastric cancer was analyzed.ROC curve was used to evaluate the diagnostic value of serum indexes combined in gastric cancer.Results The serum levels of CEA,CA724,CA19-9,and CA125 in the malignant group were higher than those in the control group(P<0.05);There was a statistically significant difference in serum CEA,CA724,CA19-9,and CA125 levels among gastric cancer patients in terms of differentiation,TNM staging,lymph node metastasis,and distant metastasis(P<0.05);Serum CEA,CA724,CA19-9,and CA125 were positively correlated with differentiation degree,TNM staging,lymph node metastasis,and distant metastasis,with statistically significant differences(P<0.05);The AUC of serum CEA,CA724,CA19-9 and CA125 combined diagnosis of gastric cancer was higher than that of each index alone diagnosis(P<0.05).Conclusions The serum CEA,CA724,CA19-9,CA125 expression is higher in patients with gastric cancer,which is related to the differentiation degree of patients,TNM stage,lymph node metastasis and distant metastasis,and the combined detection of gastric cancer has high reference value.
