前馈控制联合质量反馈方法在80岁以上长期住院患者经外周置入中心静脉导管并发症中的应用效果

Application effect of feedforward control combined with quality feedback in PICC complication in long-term hospitalized patients over 80 years old

目的观察前馈控制联合质量反馈方法在80岁以上长期住院患者经外周置入中心静脉导管(PICC)并发症中的应用效果。方法选取2019年4月至2022年12月在某三甲医院老年病房接受PICC置管的176例80岁以上患者作为研究对象,根据PICC置管前后时间顺序,将2019年4月至2021年3月置管患者共83例为对照组,2021年4月至2022年12月置管患者共83例为观察组,对照组给予置管后常规护理,观察组在置管后常规护理基础上给予前馈控制联合质量反馈管理方法。比较2组患者在PICC置管后并发症发生率和非计划拔管发生率。结果观察组患者并发症发生率为12.05%,低于对照组(25.30%),差异有统计学意义(P<0.05);观察组患者因并发症导致的非计划拔管发生率为2.41%,低于对照组(14.46%),差异有统计学意义(P<0.05)。结论前馈控制联合质量反馈的方法能够有效降低长期住院老年患者PICC置管后并发症的发生,减少因并发症导致的非计划拔管事件。

Objective To observe the effect of feedforward control combined with quality feedback in PICC complications in long-term hospitalized patients over 80 years old.Methods A total of 176 patients over 80 years old who received PICC catheterization in the geriatric ward of a tertiary hospital from April 2019 to December 2022 were selected as the study subjects.According to the time sequence before and after PICC catheterization,a total of 83 patients with catheterization from April 2019 to March 2021 were selected as the control group,a total of 83 patients with catheterization from April 2021 to December 2022 were in the observation group.The control group was given routine nursing after catheterization,and the observation group was given feedforward control combined with quality feedback management on the basis of routine nursing after catheterization.The incidence of complications and unplanned extubation after PICC catheterization were compared between the two groups.Results The incidence of complications in observation group was 12.05%,which was lower than that in control group(25.30%),the difference was statistically significant(P<0.05);the incidence of unplanned extubation due to complications in observation group was 2.41%,lower than that in control group(14.46%),the difference was statistically significant(P<0.05).Conclusions The method based on feedforward control combined with quality feedback can effectively reduce the occurrence of complications after PICC catheterization in long-term hospitalized elderly patients,and reduce the unplanned extubation events caused by complications.
