多层螺旋CT在超早期小肝癌患者中的辅助诊断效果及影像学特点研究

Study on the Auxiliary Diagnostic Effect and Imaging Characteristics of Multi-slice Spiral CT in Patients with Ultra-early Small Liver Cancer

目的:探讨多层螺旋CT在超早期小肝癌患者中的辅助诊断效果及影像学特点研究。方法:选取2021年4月至2022年4月于我院进行检查的未确诊的超早期小肝癌患者90例,随机分为两组,对照组检查方式为磁共振增强扫描,研究组检查方式为多层螺旋CT检查,对两组的检查结果进行对比。结果:未确诊90例患者中阳性患者55例,阴性患者35例。对照组敏感度为76.36%(42/55),准确率为68.89%(62/90),特异性为57.14%(20/35);研究组敏感度为94.55%(52/55),准确率为90.00%(81/90),特异性为82.86%(29/35),两组对比差异有统计学意义(P<0.05)。小肝癌病灶密度的变化形式主要为高密度、等密度、低密度的模式,其中动脉期多为高密度变化,约为96.67%,门静脉期、延迟期多为低密度变化,约为74.44%和86.67%。结论:超早期小肝癌患者应用多层螺旋CT诊断具有较高的敏感度,另外其中的多期增强扫描可以明确具体的血供变化情况,为疾病的诊断、治疗提供依据,值得推广应用。

Objective:To investigate the auxiliary diagnostic effect and imaging characteristics of multi-slice spiral CT in patients with ultra-early small liver cancer.Methods:a total of 90 undiagnosed patients with ultra-early small liver cancer who underwent examination in our hospital from April 2021 to April 2022 were randomly divided into two groups.The control group was examined by enhanced magnetic resonance scanning,and the research group was examined by multi-slice spiral CT.The results of the two groups were compared.Results:Among the 90 undiagnosed patients,55 were positive and 35 were negative.The sensitivity,accuracy and specificity of the control group were 76.36%(42/55),68.89%(62/90)and 57.14%(20/35)respectively.The sensitivity,accuracy and specificity of the study group were 94.55%(52/55),90.00%(81/90)and 82.86%(29/35),respectively,and the differences between the two groups were statistically significant(P<0.05).The density of small hepatocellular carcinoma lesions mainly changed in high-density,isodensity and low-density modes.In arterial stage,high-density changes were mostly found,accounting for 96.67%,while in portal vein stage and delayed stage,low-density changes were mostly found,accounting for 74.44%and 86.67%.Conclusion:MSCT has high sensitivity in the diagnosis of patients with ultra-early small liver cancer.In addition,MSCT can clarify the specific changes of blood supply,provide basis for the diagnosis and treatment of the disease,and is worthy of popularization and application.
