摘要
目的探讨血清补体1q/肿瘤坏死因子相关蛋白3(CTRP3)、C-X-C基序趋化因子配体14(CXCL14)水平对子宫内膜异位症(EM)术后复发的预测价值。方法选取手术治疗的EM患者132例为EM组,并根据术后3年内是否复发将患者分为复发组52例、未复发组80例;健康体检妇女76名为对照组。酶联免疫吸附法检测血清CTRP3、CXCL14;收集EM组相关资料;单因素分析与多因素Logistic回归分析血清CTRP3、CXCL14对EM患者术后复发的影响;受试者工作特征(ROC)曲线分析血清CTRP3、CXCL14水平对EM患者术后复发的预测价值。结果EM组血清CTRP3、CXCL14水平低于对照组(P均<0.05)。年龄增加、EM生育指数评分升高、CTRP3升高、CXCL14升高为EM患者术后复发的独立保护因素(P均<0.05),ASRM分期Ⅲ、Ⅳ期为独立危险因素(P<0.05)。血清CTRP3、CXCL14水平联合预测EM患者术后复发的曲线下面积为0.855,大于二者单独预测的0.764、0.769(P均<0.05)。结论血清CTRP3、CXCL14水平降低与EM患者术后复发密切相关,二者联合预测EM患者术后复发的价值较高。
Objective To investigate the predictive value of serum complement 1q/tumor necrosis factor-related protein 3(CTRP3)and C-X-C motif chemokine ligand 14(CXCL14)levels for postoperative recurrence of endometriosis(EM).Methods Totally 132 EM patients who underwent surgical treatment(EM group)and 76 healthy women with physical examination during the same period(control group)were selected,and the EM patients were divided into the recurrence group(52 cases)and the non-recurrence group(80 cases)according to whether they had recurrence after surgery or not.Serum CTRP3 and CXCL14 levels were measured by enzyme-linked immunosorbent assay.We collected the related data in the EM group.The effects of serum CTRP3 and CXCL14 on postoperative recurrence in EM patients were analyzed by Univariate analysis and Multivariate Logistic regression analysis.The predictive value of serum CTRP3 and CXCL14 levels on postoperative recurrence in EM patients was analyzed using the receiver operating characteristic(ROC)curve.Results The serum levels of CTRP3 and CXCL14 in the EM group were lower than those in the control group(all P<0.05).Increased age,increased EM fertility index score,increased CTRP3,and increased CXCL14 were independent protective factors for postoperative recurrence in EM patients(all P<0.05),and ASRM stage III-IV was an independent risk factor(P<0.05).The area under the curve of serum CTRP3 and CXCL14 levels in predicting the postoperative recurrence in EM patients was 0.855,which was greater than that of serum CTRP3 and CXCL14 levels alone(0.764 and 0.769)(all P<0.05).Conclusions Reduced serum CTRP3 and CXCL14 levels are closely associated with postoperative recurrence in EM patients,and the combination of the two is of high value in predicting postoperative recurrence in EM patients.
