立体定向穿刺引流联合高压氧治疗中等量丘脑-内囊出血预后分析

Prognostic Analysis of Stereotactic Puncture Drainage Combined with Hyperbaric Oxygen Therapy for Moderate Amounts Thalamic Internal Capsule Hemorrhage

目的探讨立体定向穿刺引流联合高压氧治疗中等量丘脑-内囊出血的临床效果。方法选取医院2020年12月至2021年12月收治的中等量丘脑-内囊区出血患者47例,按治疗方式的不同分为观察组(15例)与对照组(32例)。比较两组患者的治疗相关指标、预后指标等。结果观察组高压氧治疗开始时间明显早于对照组,神经科住院时间明显短于对照组(P<0.05)。观察组治疗后3 d、7 d、2周的残余血肿量明显少于对照组(P<0.001),且观察组重症肺部感染和消化道出血发生率明显低于对照组(P<0.05)。观察组治疗2周时的美国国立卫生研究院卒中量表(NIHSS)评分及出院6个月后的日常生活活动能力(ADL)评分均明显优于对照组(P<0.05)。结论立体定向穿刺引流联合高压氧治疗中等量丘脑-内囊出血,可有效改善患者的病情及预后。

Objective To investigate the clinical effect of stereotactic puncture drainage combined with hyperbaric oxygen therapy in the treatment of moderate amounts thalamic internal capsule hemorrhage.Methods A total of 47 patients with moderate amounts of thalamic internal capsule hemorrhage admitted to the hospital from December 2020 to December 2021 were selected and divided into the observation group(15 cases)and the control group(32 cases)according to different treatment methods.The treatment related indicators and prognostic indicators were compared between two groups of patients.Results The start time of hyperbaric oxygen treatment in the observation group was significantly earlier than that in the control group,and the hospitalization time in the neurology department was significantly shorter than that in the control group(P<0.05).The residual hematoma volume in the observation group were significantly lower than those in the control group at 3 d,7 d,and 2 weeks after treatment(P<0.001),and the incidences of severe pulmonary infection and gastrointestinal bleeding in the observation group were significantly lower than those in the control group(P<0.05).The National Institutes of Health Stroke Scale(NIHSS)score at 2 weeks of treatment and the Activities of Daily Living(ADL)score at 6 months after discharge in the observation group were significantly better than those in the control group(P<0.05).Conclusion The combination of stereotactic puncture drainage and hyperbaric oxygen therapy can effectively improve the condition and prognosis of patients with moderate amounts thalamic internal capsule hemorrhage.
