针刺结合引火归元穴位贴敷治疗上实下虚型颈性眩晕临床观察

Acupuncture Combined with Acupoint Application for Returning Fire to its Origin in the Treatment of Cervical Vertigo of Upper Excess and Lower Deficiency Differentiation

目的观察针刺结合引火归元穴位贴敷治疗上实下虚型颈性眩晕的临床效果。方法选取120例上实下虚型颈性眩晕患者,按随机数字表法分为针刺组、引火归元组、针刺+引火归元组与西药组并行相应治疗,每组30例。比较四组治疗前后颈性眩晕症状与功能评估量表评分、血清内皮型一氧化氮合酶(eNOS)及人降钙素基因相关肽(CGRP)水平变化,治疗结束后3个月进行回访比较复发率。结果治疗后,四组颈性眩晕症状与功能评估量表评分均较治疗前显著升高,且针刺+引火归元组升高幅度大于其余三组(P<0.05);四组血清eNOS、CGRP水平均较治疗前显著升高,且针刺+引火归元组升高幅度大于其余三组。治疗结束3个月后,针刺+引火归元组复发率为26.67%(8/30),低于其余三组。结论常规针刺、引火归元穴位贴敷、针刺结合引火归元穴位贴敷与口服甲磺酸倍他司汀片等方法对上实下虚型颈性眩晕患者均疗效确切,且针刺结合引火归元穴位贴敷效果最优、复发率最低,值得推广。

Objective To observe the clinical efficacy of acupuncture combined with acupoint application for returning fire to its origin in the treatment of cervical vertigo of upper excess and lower deficiency differentiation.Methods A total of 120 patients with cervical vertigo of upper excess and lower deficiency differentiation were randomly divided into acupuncture group,acupoint application for returning fire to its origin group,acupuncture combined with acupoint application group and Western medicine group,with 30 cases in each.Before and after treatment,patients were observed by Cervical Vertigo Symptoms and Function Assessment Scale,and their level of eNOS and CGRP were tested.Three months after the end of treatment,the recurrence rates of the four groups were followed up.Results After treatment,the scores of cervical vertigo symptom and functional assessment scale in the four groups were significantly higher than those before treatment,and the increases in acupuncture combined with acupoint application for returning fire to its origin group were greater than those in the other three groups(P<0.05).Serum eNOS and CGRP levels in the four groups were significantly higher than those before treatment,and the increases in the acupuncture combined with acupoint application for returning fire to its origin group were greater than those in the other three groups.Three months after the end of treatment,the recurrence rate of acupuncture combined with acupoint application for returning fire to its origin group was 26.67%(8/30),which was lower than the other three groups.Conclusion All four kinds of method are effective for patients with cervical vertigo of upper excess and lower deficiency differentiation,while the efficacy of acupuncture combining acupoint application for returning fire to its origin is more significant than the others,and the recurrence rate is lower.
