摘要
目的探讨99mTc-MDP全身骨断层融合显像半定量技术对前列腺癌骨转移病灶识别的价值。方法选择我院于2021年5月~2023年5月前列腺癌患者80例,术后病理显示前列腺癌骨转移患者54例,无骨转移患者26例。所有患者均行99mTc-MDP全身骨断层融合显像半定量技术检查。由1名主治医师和1名主任医师独立完成图像分析。经QMetrix分析即可获得平均标准化摄取值(SUV_(mean))和最大标准化摄取值(SUV_(max))。比较骨转移组与无骨转移组血清前列腺特异性抗原(PSA),SUV_(mean)和SUV_(max)变化;分析99mTc-MDP全身骨断层融合显像诊断骨转移准确率、灵敏度和特异度;Pearson分析PSA与SUV_(mean)和SUV_(max)相关性。结果骨转移组血清PSA水平高于无骨转移组(P<0.05)。骨转移组SUV_(mean)和SUV_(max)值大于无骨转移组(P<0.05)。99mTc-MDP全身骨断层融合显像诊断骨转移准确率为88.89%(48/54)。99mTc-MDP全身骨断层融合显像诊断骨转移灵敏度为79.63%,特异度为80.77%。经Pearson相关性分析显示,PSA与SUV_(mean)和SUV_(max)呈线性正相关(P<0.05)。结论99mTc-MDP全身骨断层融合显像半定量技术对前列腺癌骨转移病灶诊断准确率较高,且具有良好诊断灵敏度和特异度,值得临床借鉴。
Objective Explore the value of semi quantitative 99mTc MDP whole body bone tomographic fusion imaging in identifying bone metastases from prostate cancer.Methods 80 patients with prostate cancer were selected from our hospital from May 2021 to May 2023.Postoperative pathology showed 54 patients with prostate cancer bone metastasis and 26 patients without bone metastasis.All patients underwent 99mTc MDP whole body bone tomographic fusion imaging with semi quantitative technique.The image analysis was independently completed by one attending physician and one Chief physician.The average standardized uptake value(SUV_(mean))and maximum standardized uptake value(SUV_(max))can be obtained through QMetrix analysis.Compare the changes in serum prostate specific antigen(PSA),SUV_(mean),and SUV_(max)between the bone metastasis group and the non bone metastasis group;To analyze the accuracy,Sensitivity and specificity of 99mTc MDP whole body bone tomography fusion imaging in the diagnosis of bone metastasis;Pearson analyzed the correlation between PSA and SUV_(mean)and SUV_(max).Results The bone metastasis group serum PSA level higher than non bone metastasis group(P<0.05).The bone metastasis group SUV_(mean)and SUV_(max)values higher than non bone metastasis group(P<0.05).The accuracy of 99mTc MDP whole body bone tomographic fusion imaging in diagnosing bone metastasis was 88.89%(48/54).The sensitivity and specificity of 99mTc-MDP whole body bone tomographic fusion imaging in diagnosing bone metastasis were 79.63%and 80.77%,respectively.Pearson correlation analysis showed that PSA showed a linear positive correlation with SUV_(mean)and SUV_(max)(P<0.05).Conclusion The semi quantitative technique of 99mTc MDP whole body bone tomographic fusion imaging has a high diagnostic accuracy for bone metastases of prostate cancer,and has good Sensitivity and specificity,which is worthy of clinical reference.
