磁性附着体在下颌种植覆盖义齿修复中对咀嚼效率与固位功能的影响

摘要 目的探讨磁性附着体在下颌种植覆盖义齿修复中对咀嚼效率与固位功能的影响。方法选取2019年9月至2021年1月就诊的130例下颌牙列缺失患者,依据下颌种植覆盖义齿修复治疗方法的不同分为2组,采用磁性附着体修复的65例患者作为磁性组,采用球帽附着体修复的65例患者作为球帽组。对比2组患者覆盖义齿修复后咀嚼效率、自觉满意情况、种植体周围软组织健康状况、种植体骨吸收量及美学修复效果。结果覆盖义齿修复后6个月及12个月,2组咀嚼效率组内及组间比较差异无统计学意义(P>0.05)。覆盖义齿修复后12个月,磁性组患者舒适度和固位功能评分较球帽组患者高(P<0.05)。覆盖义齿修复后12个月,磁性组患者牙周袋探诊深度和牙龈指数评分较球帽组患者低(P<0.05)。覆盖义齿修复后6个月及12个月,2组患者种植体骨吸收量组内及组间比较差异均无统计学意义(P>0.05)。覆盖义齿修复后12个月,磁性组患者面部形态满意率、无染色率和边缘适宜率均较球帽组患者高(P<0.05)。结论磁性附着体在下颌种植覆盖义齿修复中应用效果佳,咀嚼效率及固位功能良好,同时种植体周围软组织健康状况较好,且不会引起患者骨吸收,安全性及美学效果较好,患者满意度高。 Objective To investigate the effects of magnetic attachments on masticatory efficiency and retention func-tion in restoration with mandibular implant-supported overdenture.Methods A total of 130 patients with mandibular edentu-lous jaw treated in the Department of Stomatology from September 2019 to January 2021 were selected and divided into two groups according to the different restoration and fixation methods of mandibular implant-supported overdenture.Sixty-five pa-tients treated with magnetic attachment were enrolled as the magnetic group and 65 patients treated with ball-cap attachment were enrolled as the ball-cap group.The masticatory efficiency,self-satisfaction,soft tissue health around the implant,im-plant bone absorption and aesthetic repair effect were compared between the two groups.Results At 6 and 12 months after overdenture restoration,there was no significant intragroup or intergroup difference in masticatory efficiency in the two groups(P>0.05).At 12 months after the overdenture restoration,the scores of comfort and retention function of the magnetic group were higher than those of the ball-cap group(P<0.05).At 12 months after overdenture restoration,periodontal pocket pro-bing depth and gingival index scores in the magnetic group were lower than those in the ball-cap group(P<0.05).At 6 months and 12 months after overdenture restoration,there was no significant intragroup or intergroup difference in implant bone absorption(P>0.05).At 12 months after the overdenture restoration,satisfaction rate of the facial morphology,no staining rate and edge suitability rate in the magnetic group were higher than those in the ball-cap group(P<0.05).Conclusion The application of magnetic attachment in restoration with mandibular implant-supported overdenture has good effects,good masticatory efficiency and retention function,and good soft tissue health around the implant,without causing bone resorption,and with good safety and aesthetic effects,and high patient satisfaction.
