微创钢板内固定术与切开复位内固定术治疗肋骨骨折患者的效果分析

Effect Analysis of Minimally Invasive Plate Internal Fixation and Open Reduction Internal Fixation in the Treatment of Patients with Rib Fracture

摘要 目的:探究微创钢板内固定术与切开复位内固定术治疗肋骨骨折患者的效果。方法:选择2022年3月—2024年2月枣庄市峄城区人民医院收治的82例肋骨骨折患者为研究对象,按随机数字法将其分为对照组和研究组,各41例。对照组采用切开复位内固定术治疗,研究组采用微创钢板内固定术治疗。对比两组围手术期临床指标、疼痛程度、肺功能指标及并发症发生情况。结果:与对照组相比,研究组手术时间、呼吸机使用时间均短于对照组,术后出血量、胸腔引流量均低于对照组,切口长度短于对照组,差异均有统计学意义(P<0.05)。术后14 d,两组第1秒用力呼气容积、用力肺活量、最大呼气峰流速均较术前升高,且研究组高于对照组,差异均有统计学意义(P<0.05)。术后3、7、14 d,研究组疼痛视觉模拟评分均低于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率为2.44%,低于对照组的19.51%,差异有统计学意义(P<0.05)。结论:微创钢板内固定术相较于切开复位内固定术,能提升手术质量,缩短肋骨骨折患者的术后康复周期,改善其肺功能状态,降低并发症发生率。 Objective:To explore the efficacy of minimally invasive plate internal fixation and open reduction internal fixation in the treatment of patients with rib fracture.Methods:A total of 82 patients with rib fracture treated in Yicheng District People’s Hospital of Zaozhuang City from March 2022 to February 2024 were selected as the study objects.They were divided into a control group and a study group by random number method,with 41 cases in each group.The control group was treated with open reduction and internal fixation,the study group was treated with minimally invasive plate and internal fixation.The perioperative clinical indexes,pain degree,pulmonary function indexes and complications were compared between the two groups.Results:Compared with the control group,the operation time and ventilator use time of the study group were shorter than those of the control group,the postoperative blood loss and thoracic drainage flow were lower than those of the control group,the incision length was shorter,with statistical significance(P<0.05).On the 14th day after surgery,forced expiratory volume,forced vital capacity and maximum peak expiratory flow rate in the first second of both groups were higher than those before surgery,the study group was higher than the control group,the differences were statistically significant(P<0.05).At 3,7 and 14 days after surgery,the visual analogue scores of the study group were lower than those of the control group,with statistical significance(P<0.05).The complication rate of the study group was 2.44%,which was lower than 19.51%of the control group,the difference was statistically significant(P<0.05).Conclusion:Compared with open reduction and internal fixation,minimally invasive plate internal fixation can improve the surgical quality,shorten the postoperative rehabilitation period,improve lung function and reduce the incidence of complications in patients with rib fracture.
