三维可视化结合可弯鞘技术在鹿角形肾结石FURS术中的研究

The study of three-dimensional visualization combined with bendable sheath technology in FURs surgery for staghorn shaped kidney stones

目的 探究三维可视化结合可弯鞘技术在鹿角形肾结石(SC)输尿管软镜钬激光碎石术(FURS)中的应用价值.方法选取2022年4月-2024年1月本院收治的80例SC患者,以随机数字表法分为两组,即对照组(n=40)、研究组(n=40),对照组予以微创通道经皮肾镜联合FURS术治疗,研究组予以三维可视化结合可弯鞘技术辅助FURS术治疗;术后均获得12周随访结果.比较两组手术有关指标、结石清除率(术后3d、术后4周、术后12周)、术后并发症发生率.结果 与对照组比较,研究组手术用时、住院时间均更短,术中出血量更少,平均穿刺通道深度更短,有统计学差异(P<0.05);研究组术后3d、4周的结石清除率高于对照组(P<0.05);两组术后12周时比较,无统计学差异(P>0.05);与对照组比较,研究组并发症发生率更低,有统计学差异(P<0.05).结论 三维可视化结合可弯鞘技术辅助FURS术应用于SC患者中,可缩短手术时间,缩短穿刺通道深度,提高结石清除率,降低并发症风险,进而促进患者恢复.

Objective To explore the application value of three-dimensional visualization combined with bendable sheath technology in ureteral flexible holmium laser lithotripsy(FURS)for staghorn shaped kidney stones(SC).Methods Eighty SC patients admited to our hospital from April 2022 to January 2024 were selected and randomly divided into two groups using a random number table method:the control group(n=40)and the study group(n=40).The control group received minimally invasive percutaneous nephroscopy combined with FURS surgery,while the study group received three-dimensional visualization combined with flexible sheath surgery to assist with FURS surgery;all patients were followed up for 12 weeks after surgery.Compare two groups of surgical related indicators,stone clearance rate(postoperative 3 days,postoperative 4 weeks,postoperative 12 weeks),and incidence of postoperative complications.Results Compared with the control group,the study group had shorter surgical time and hospital stay,less intraoperative bleeding,and shorter average puncture depth,with statistical differences(P<0.05);the stone clearance rate in the study group was higher than that in the control group at 3 days and 4 weeks after surgery(P<0.05);there was no statistically significant difference(P>0.05)between the two groups at 12 weeks after surgery;compared with the control group,the incidence of complications in the study group was lower,with a statistical difference(P<0.05).Conclusion The combination of 3D visualization and flexible sheath technology assisted FURS surgery in SC patients can shorten the surgical time,shorten the depth of the puncture channel,improve the stone clearance rate,reduce the risk of complications,and promote patient recovery.
