侧后路经皮脊柱内镜联合独活寄生汤对腰椎间盘突出症患者伤椎影像学指标的影响

Effect of Lateral Posterior Percutaneous Spinal Endoscopy Combined with Duhuo Jisheng Decoction on Imaging Indexes of Injured Vertebrae in Patients with Lumbar Intervertebral Disc Protrusion

目的:研究侧后路经皮脊柱内镜联合独活寄生汤对腰椎间盘突出症(LIDP)患者伤椎影像学指标的影响。方法:选取2016年1月—2018年12月在我院接受治疗的90例LIDP患者作为观察对象,采用随机数字表法分为A组和B组,各45例。A组采用侧后路经皮脊柱内镜联合独活寄生汤治疗,B组采用侧后路经皮脊柱内镜治疗。比较两组治疗有效率、影像学指标、炎性指标以及视觉模拟疼痛评分情况。结果:A组治疗有效率高于B组,差异有统计学意义(P<0.05);治疗后A组根袖直径和ASLR影像学指标均高于B组,差异有统计学意义(P<0.05);治疗后A组血清IL-6、IL-8及VAS评分均低于治疗前,且低于B组,差异有统计学意义(P<0.05)。结论:侧后路经皮脊柱内镜治疗的基础上联合独活寄生汤治疗LIDP的疗效显著,能显著改善患者的影像学指标,降低炎性指标和疼痛程度,值得应用和推广。

Objective:To study the effect of lateral posterior percutaneous spinal endoscopy combined with duhuo jisheng decoction on imaging indexes of injured vertebrae in patients with lumbar intervertebral disc protrusion(LIDP).Methods:90 patients with LIDP who were treated in our hospital from January 2016 to December 2018 were selected as subjects observed,and they were randomly divided into group A and group B,45 cases each.The patients in the group A were treated with lateral posterior percutaneous spinal endoscopy combined with duhuo jisheng decoction,and the patients in the group B were treated with lateral posterior percutaneous spinal endoscopy.The therapeutic effective rate,imaging indexes,inflammatory indexes and visual analogue pain score were compared between the two groups.Results:The therapeutic effective rate in the group A was higher than that in the group B,and the difference was statistically significant(P<0.05).After treatment,the diameter of the root sleeve and ASLR imaging indexes in the group A were higher than those in the group B,and the differences were statistically significant(P<0.05).The levels of serum IL-6,IL-8 and VAS score after treatment were lower than those before treatment in the group A,and the levels in the group A were lower than those in group B,with statistically significant differences(P<0.05).Conclusion:Duhuo jisheng decoction based on lateral posterior percutaneous spinal endoscopy has a significant therapeutic effect on LIDP,which can significantly improve the imaging indexes of the patients and reduce inflammatory indexes and pain degree,and is worthy of application and popularization.
