无创呼吸机联合纳洛酮治疗慢性阻塞性肺疾病合并重症呼吸衰竭的临床疗效 被引量:9

Clinical Efficacy of Noninvasive Ventilators Combined with Naloxone in the Treatment of Chronic Obstructive Pulmonary Diseases Complicated with Severe Respiratory Failure

摘要 目的:探讨无创呼吸机联合纳洛酮治疗慢性阻塞性肺疾病(COPD)合并重症呼吸衰竭的临床疗效。方法:选取2017年2月—2018年2月收治的COPD合并重症呼吸衰竭患者96例,按随机数字表法分为两组,各48例。两组患者均进行常规治疗,对照组在常规治疗基础上给予无创呼吸机辅助治疗,研究组在对照组基础上联合纳洛酮(1.8 mg)治疗。结果:研究组总有效率高于对照组,差异有统计学意义(P<0.05);研究组血液酸碱度(pH值)、血氧分压(PaO2)高于对照组,血氧二氧化碳(PaCO2)低于对照组,差异有统计学意义(P<0.05);研究组肺总容量(TLC)、第一秒用力呼气容积(FEV1)、呼气流量(PEF)25、PEF50指标高于对照组,差异有统计学意义(P<0.05)。结论:无创呼吸机联合纳洛酮治疗COPD合并重症呼吸衰竭的临床效果更加显著,可有效提升患者的血氧分压,降低高碳酸血症,纠正呼吸衰竭,值得推广。 Objective:To explore the clinical efficacy of noninvasive ventilators combined with naloxone in the treatment of chronic obstructive pulmonary diseases(COPD)complicated with severe respiratory failure.Methods:96 patients with COPD complicated with severe respiratory failure admitted to our hospital from February 2017 to February 2018 were selected and randomly divided into two groups:a control group and a study group,48 cases each.The patients in the two groups were treated conventionally.The patients in the control group were given noninvasive ventilators-assisted treatment on the basis of the conventional treatment.The patients in the study group were treated with naloxone(1.8 mg)on the basis of the treatment in the control group.Results:The total effective rate in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).The blood pH value and partial pressure of blood oxygen(PaO2)in the study group were higher than those in the control group,while the partial pressure of blood carbon dioxide(PaCO2)was lower than that in the control group,with statistically significant differences(P<0.05).The total lung volume(TLC),forced expiratory volume in the first second(FEV1),peak expiratory?ow(PEF)25 and PEF50 indexes in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Noninvasive ventilators combined with naloxone have a more significant clinical effect on COPD complicated with severe respiratory failure,which can effectively increase the partial pressure of blood oxygen of the patients,alleviate hypercapnia and correct respiratory failure,and is worth of popularization.
