摘要 目的:探讨尿素(Urea)、微量白蛋白(UAlb)、β2-微球蛋白(U-β2MG)及胱抑素(Cys)-C水平变化与糖尿病肾病患者病情严重程度的关系。方法:2016年6月—2018年6月我院内分泌科收治的136例2型糖尿病患者,依据尿蛋白含量分为糖尿病组(n=50)和糖尿病肾病组(n=86),糖尿病肾病组依据肾功能和肾小球滤过率分为早期糖尿病肾病组、临床期糖尿病肾病组及晚期糖尿病肾病组,比较各组尿液中Urea、UAlb、U-β2MG及Cys-C水平及各指标阳性率。结果:糖尿病肾病组尿液中Urea、UAlb、U-β2MG及Cys-C水平高于糖尿病组,差异有统计学意义(P<0.05);随着糖尿病肾病加重,患者Urea、UAlb、U-β2MG及Cys-C水平显著上升,各指标异常率也逐渐升高,差异有统计学意义(P<0.05)。结论:糖尿病肾病患者尿液中Urea、UAlb、U-β2MG及Cys-C水平异常,且随病情加重而升高,可作为糖尿病患者严重微血管病变的监测指标。 Objective:To investigate the relationship between the changes of urea(Urea),microalbumin(UAlb),β2-microglobulin(U-β2 MG)and Cystatin(Cys)-C levels and the severity of diabetic nephropathy.Methods:136 patients with type 2 diabetes mellitus admitted to the Department of Endocrinology in our hospital from June 2016 to June 2018 were divided into a diabetes mellitus group(n=50)and a diabetic nephropathy group(n=86)according to urine protein contents.The patients in the diabetic nephropathy group were divided into an early diabetic nephropathy group,a clinical diabetic nephropathy group and an advanced diabetic nephropathy group according to renal functions and glomerular filtration rate.The levels of Urea,UAlb,U-β2 MG and Cys-C and the positive rate of each index in urine were compared between the two groups.Results:The levels of urine Urea,UAlb,U-β2 MG and Cys-C the diabetic nephropathy group were higher than those in the diabetes mellitus group,and the differences were statistically significant(P<0.05).The levels of Urea,UAlb U-β2 MG and Cys-C in the patients increased significantly with the aggravation of diabetic nephropathy,and the abnormal rate of each index also increased gradually,with statistically significant differences(P<0.05).Conclusion:The levels of Urea,UAlb,U-β2 MG and Cys-C in urine of the patients with diabetic nephropathy are abnormal,and increase with the aggravation of the disease,which can be used as monitoring indexes for severe microvascular diseases in diabetic patients.
