湿润烧伤膏联合蚕食性清创治疗糖尿病足患者的临床研究

Study on Clinical Therapeutic Effect of Moist Exposed Burn Ointment Combined with Necrotic Debridement on Patients with Diabetic Feet

目的:探讨湿润烧伤膏联合蚕食性清创治疗糖尿病足的应用价值。方法:按照随机数字表法将我院2016年12月—2018年8月收治的88例糖尿病足患者分为观察组与对照组,各44例。两组均予以常规全身治疗与蚕食性清创,清创后对照组给予外用冻干重组人酸性成纤维细胞生长因子(rh-αFGF)粉剂,观察组在此基础上联合湿润烧伤膏,连续治疗5周后评价疗效。比较两组治疗总有效率、创面愈合时间、治疗前后生活质量(SF-36评分)、治疗期间不良反应发生率。结果:观察组治疗总有效率高于对照组,创面愈合时间短于对照组,差异有统计学意义(P<0.05);治疗5周后观察组SF-36各维度评分高于对照组,差异有统计学意义(P<0.05);治疗期间,两组均无严重不良反应发生。结论:湿润烧伤膏联合蚕食性清创治疗糖尿病足的效果显著,有利于缩短创面愈合时间,改善患者的生活质量,且安全性高。

Objective:To explore the application value of moist exposed burn ointment(MEBO)combined with necrotic debridement in the treatment of diabetic feet.Methods:88 patients with diabetic feet admitted to our hospital from December 2016 to August 2018 were selected and divided into an observation group and a control group,44 cases each.The patients in two groups were given conventional systemic therapy and necrotic debridement.After debridement,the patients in the control group were given lyophilized recombinant human acidic fibroblast growth factor(rh-αFGF)powder,and the patients in the observation group were further treated with MEBO.The efficacy was evaluated after 5 weeks of continuous treatment.The total effective rate,wound healing time,quality of life(SF-36 score)before and after treatment and the incidence of adverse reactions during treatment were compared between the two groups.Results:The total effective rate in the observation group was higher than that in the control group,and the wound healing time was shorter than that in the control group,with statistically significant differences(P<0.05).After 5 weeks of treatment,the SF-36 scores in different dimensions in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).No serious adverse reactions occurred in the two group during the treatment period.Conclusion:MEBO combined with necrotic debridement has a significant therapeutic effect on diabetic feet,which can shorten the wound healing time and improve the quality of life of patients,and has high safety.
