浅析蒙医药治疗类风湿关节炎临床方药特点

Analysis of Clinical Characteristics of Mongolian Medicine in the Treatment of Rheumatoid Arthritis

目的:基于数据挖掘技术对蒙古族治疗类风湿关节炎临床方药进行归纳总结,分析蒙古族医药治疗类风湿关节炎的用药规律和特色。方法:收集《蒙医成方选》《传统蒙药与方剂》《名老蒙医高喜验方》有关治疗类风湿关节炎的方药,构建方药数据库,利用Microsoft Excel 2019对药物性味、主治功用进行频次分析,通过SPSS Statistics 21.0、SPSS Modeler 14.0对处方药物进行系统聚类、关联规则及主成分因子分析。结果:(1)共收集蒙古族治疗类风湿关节炎的成方共有142首,使用药物258味,用药总频次达2051次;方药剂型多以丸剂,且用药以植物药居多;药效以燥、钝、重为主;药性以温、凉药居多;五味中辛、苦类药物最为常用;(2)系统聚类得到药物核心组合5类;(3)关联规则挖掘出2味药有效组合18组,3味药有效组合7组;蒙古族治疗类风湿关节炎以枫香脂、决明子、并头黄芩、诃子为核心药物,挖掘出三子散、枫香脂十味丸、四味决明子散和嘎日迪-5味丸四个基本处方;(4)因子分析提取出8个公因子。结论:蒙医治疗类风湿关节炎用药有其自身的特色,清热、杀黏、燥协日乌素(黄水)贯彻类风湿关节炎全过程,但如何将蒙医理论阐述的治疗原则与中医医理、现代药理等联系起来进行深入研究开发,有待进一步探讨。

Objective:Based on the data mining,this article summarized the clinical formulas of Mongolian treating Rheumatoid Arthritis,and analyzed the medication rules and characteristics.Methods:Mongolian Medicine prescriptions for the rheumatoid arthritis were collected from Selection of Mongolian Prescription,Traditional Mongolian Medicine and Prescription and Famous Mongolian Medicine Gao Xi Prescription,to build a prescription database.Microsoft Excel 2019 was used to analyze the frequency of medicine properties,flavors and indications.The systematic clustering,the association rules and the principal component factor analysis of prescription drugs were performed by SPSS Statistics 21.0 and SPSS Modeler 14.0.Results:(1)A total of 142 Mongolian prescriptions were collected for the treatment of Rheumatoid Arthritis,including 258 drugs,with a total frequency of 2051.Pills were used in the most formulations,and drugs derived from plants were in the majority.The efficacies were mainly Zao,Dun and Zhong.The properties of medicine were mainly Wen and Liang.The most commonly used flavors were Ku and Xin drugs.(2)5 core drug combinations were obtained by systematic clustering.(3)The systematic clustering results showed that there were 18 groups of the effective two-drug combinations,while there were 7 groups of the effective three-drug combinations.In the treatment of Rheumatoid Arthritis,maple balsam,cassia seed,scutellaria baicalensis georgi were 4 core drugs in Mongolian medicine,and 4 basic prescriptions were Sanzi Powder,Fengxiangzhishiwei Pill,Siweijuemingzi Powder and Garidiwuwei Pill.(3)8 common factors were extracted in Factor analysis.Conclusion:Mongolian medicine has its own characteristics of treating Rheumatoid Arthritis.During the whole treatment process for Rheumatoid Arthritis,the approaches of Qingre,Shanian,and Zao Xieriwusu were carried out.However,the research of medication rules of Mongolian medicine,the principles of traditional Chinese medicine and modern pharmacology needs further discussion.
