儿童呼吸道EB病毒感染的抗体特点与不同季节相关性研究 被引量:4

摘要 目的通过分析儿童呼吸道感染EB病毒(EBV)患者血清抗体特点得出不同感染情况下的临床特征。方法采用荧光免疫法检测焦作市妇幼保健院儿科于2019年5月至2020年4月收治723例疑似EB感染患儿血浆EBV抗体(抗VCA-IgG、抗NA-IgG、抗VCA-IgM、抗EA-IgG),血清抗体特点,并结合患儿临床资料进行统计学分析。结果0~3岁患儿NA-IgG和VCA-IgG阳性检出率均低于4~7岁、8~15岁组(χ^2=141.5,P<0.001;χ^2=44.4,P<0.001),但VCA-IgM阳性检出率最高,达21.6%(χ^2=6.245,P=0.044)。其中VCA-IgG和VCA-IgM阳性检出率均以春季最高(56.2%,25.8%)。感染类型结果显示0~3岁就诊患儿发生急性感染以冬季最高(15.4%,14/64),夏季最低(9.8%,10/103)。4~7岁就诊患儿发生急性感染以春、冬季最高(15.6%,7/45;16.4%,10/61)。8~15岁就诊患儿发生急性感染以春季最高(19.0%,4/21)。结论原发感染EBV易感人群为0~3岁婴幼儿,0~3岁组VCA-IgM阳性检出率最高。NA-IgG和VCA-IgG阳性检出率随着年龄增长而升高,EA-IgG在各年龄组抗体阳性检出率最低。不同季节进行统计分析,儿童EBV感染多发于春季。 Objective:To analyze the characteristics of serum antibody in children with respiratory infection of EB virus(EBV)and present clinical symptoms under different infection conditions.Methods:723 children with suspected EB infection were treated in department of pediatrics of Jiaozuo Health Center for Women and Children from January 2018 to December 2019,and fluorescence immunoassay was adopted to detect the plasma EBV antibody(anti-VCA-IgG,anti-NA-IgG,anti-VCA-IgM and anti-EA-IgG)and serum antibody.Statistical analysis was conducted in combination with clinical data of the children.Results:The positive rates of na-igg and vca-igg in children aged 0~3 years were lower than those in children aged 4~7 years and 8~15 years(χ~2=141.5,P<0.001),χ~2=44.4,P<0.001),but the highest detection rate of vca-igm was 21.6%(2=6.245,P=0.044).while the positive rate of VCA-IgM ranked at the top at 21.6%(χ~2=6.245,P=0.044).The positive rates of both VCA-IgG and VCA-IgM were the highest in spring(56.2%,25.8%).The infection type results indicated that the incidence of acute infection ranked the highest in winter(15.4%,28/128)and the lowest in summer(9.2%,19/207)in children between 0-3 years old.The incidence of acute infection ranked the highest in both spring and winter(15.7%,14/89;15.7%,19/121)in children between 4-7 years old.The incidence of acute infection ranked the highest in spring(17.1%,7/41)in children between 8-15 years old.Conclusion:The susceptible population of primary EBV was infants 0-3 years of age,and the positive rate of VCA-IgM ranked at the top in 0-3 years old group.NA-IgG and VCA-IgG as long-term in vivo antibodies last for a long time,so the positive rate shall be increased with the growing of age,while the positive rate of EA-IgG was the lowest in groups of all age.Statistical analysis in different seasons revealed that the incidence of EBV infection in children was more common in spring.
