摘要
目的了解深圳地区多囊卵巢综合征(PCOS)患者25-羟基维生素D3(25-OH-VitD3)水平及其受体(VDR)基因TaqⅠrs731236T/C位点单核苷酸多态性(SNPs)分布特征,并探讨其多态性与25-OH-VitD3和转化生长因子-β1(TGF-β1)水平之间的相关性。方法收集2017年6月~2019年3月来医院妇产门诊就诊并确诊为PCOS患者175例为PCOS组,选择同期非PCOS健康育龄妇女120名为HC组,分别采用化学发光法检测25-OH-VitD3水平和酶联免疫吸附法(ELlSA)检测TGF-β1水平,同时应用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)法检测VDR基因TaqⅠrs731236位点SNPs多态性。结果PCOS患者25-OH-VitD3水平为27.92±5.38nmol/L,明显低于HC组的49.65±10.74nmol/L,而TGF-β1水平为81.63±9.04ng/L,明显高于HC组的30.49±2.90ng/L,差异有统计学意义(P<0.05);PCOS患者VDR基因TaqⅠrs731236T/C位点CC基因型及C等位基因检出率分别为38.29%和60.57%,明显高于HC组的17.50%和37.08%,差异有统计学意义(P<0.05);VDR基因TaqⅠrs731236T/C位点CC基因型的PCOS患者血清中25-OH-VitD3水平为14.86±3.52nmol/L,明显低于TC和TT基因型的31.79±4.71nmol/L和43.67±8.64nmol/L,而TGF-β1水平为107.34±12.67 ng/L,明显高于TC和TT基因型的72.51±8.63 ng/L和48.92±5.44 ng/L,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,PCOS患者25-OH-VitD3水平与TGF-β1水平呈明显的负相关(r=-0.7364,P<0.05)。结论PCOS患者25-OH-VitD3水平明显降低,而TGF-β1水平明显升高,两者呈负相关。同时VDR基因TaqⅠrs731236位点CC基因型及C等位基因检出率明显升高,且不同基因型的PCOS患者25-OH-VitD3和TGF-β1水平之间存在差异,其中CC基因型可能与深圳地区PCOS发病有密切相关。
Objective:To investigate the distribution characteristics of 25-hydroxyl vitamin D3(25-OH-VitD3)and its receptor(VDR)gene Taq I rs731236T/C single nucleotide polymorphism(SNPs)in patients with polycystic ovary syndrome(PCOS)in shenzhen,and to explore the correlation between the polymorphism and 25-OH-VitD3 and transformed growth factor-β1(TGF-β1)levels.Methods:175 patients diagnosed with PCOS who visited the obstetrics and gynecology clinic of the hospital from June 2017 to March 2019 were selected as PCOS group,120 healthy women of non-PCOS reproductive age were selected as HC group.Chemiluminescence was used to detect the level of 25-oh-vitd3 and ELlSA was used to detect the level of TGF-β1,at the same time,application of polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)system method to detect VDR gene Taq I rs731236 loci SNPs polymorphism.Results:The level of 25-OH-VitD3 in PCOS patients was 27.92±5.38nmol/L,significantly lower than 49.65±10.74nmol/L in HC group,while the level of TGF-β1 was 81.63±9.04ng/L,significantly higher than 30.49±2.90ng/L in HC group,the difference was statistically significant(P<0.05).The detection rates of CC genotype and C allele of VDR gene Taq I rs731236T/C site in PCOS patients were 38.29%and 60.57%,respectively,which were significantly higher than 17.50%and 37.08%of HC group,and the difference was statistically significant(P<0.05).The level of 25-OH-VitD3 in PCOS patients with VDR gene Taq I rs731236T/C CCgenotype was 14.86±3.52nmol/L,significantly lower than 31.79±4.71nmol/L and 43.67±8.64nmol/L of TC and TT genotypes,while the TGF-β1 level was 107.34±12.67 ng/L,significantly higher than 72.51±8.63 ng/L and 48.92±5.44 ng/L of TC and TT genotypes.The differences were statistically significant(P<0.05).Pearson correlation analysis showed that PCOS patients had a significant negative correlation between 25-OH-VitD3 and TGF-β1 level(r=-0.7364,P<0.05).Conclusion:The level of 25-OH-VitD3 had a significant decrease in PCOS patients,while TGF-β1 was significantly increased,and the two were negatively correlated.At the same time,the detection rates of CC genotype and C allele of VDR gene Taq I rs731236 were significantly increased,and the levels of 25-OH-VitD3 and TGF-β1 in PCOS patients with different genotypes were different,among which CC genotype might be closely related to the incidence of PCOS in shenzhen.
