妊娠性高血压患者血清miRNA-17、miRNA-181b的水平与血压及妊娠结局关系的研究 被引量:5

Correlations of serum miRNA-17,miRNA-181b concentration with blood pressure and pregnancy outcome in hypertensive disorder complicating pregnancy

摘要 目的探讨妊娠性高血压患者血清miRNA-17、miRNA-181b的水平与血压及妊娠结局关系。方法选择2016年2月-2019年2月邢台医学高等专科学校第二附属医院产科住院的患者200例,根据第8版妇产科教材诊断标准将入选者分A组:早期产妇39例,B组:晚期产妇41例,C组:妊娠高血压产妇49例,D组:轻度子痫前期产妇41例,E组:重度子痫前期产妇30例。检测各组患者血清中miRNA-17、miRNA-181b表达量,并对各组血压、新生儿体重不良妊娠结局进行统计。结果E组在收缩压、舒张压、尿蛋白、miRNA-17水平、miRNA-181b水平均明显高于其余四组(P<0.05);D组收缩压、舒张压、尿蛋白、miRNA-17水平、miRNA-181b水平均明显高于C组、B组、A组(P<0.05);C组收缩压、舒张压、尿蛋白、miRNA-17水平、miRNA-181b水平均明显高于B组、A组(P<0.05)。E组羊水污染、子痫、胎儿死亡、新生儿窒息、胎儿宫内窘迫比例明显高于其余四组(P<0.05)。E新生儿出生体重比例明显低于其余四组(P<0.05)。妊娠性高血压患者血清miRNA-17水平、miRNA-181b水平与血压呈正相关(P<0.05),与新生儿出生体重呈负相关(P<0.05)。结论妊娠性高血压患者血清miRNA-17水平、miRNA-181b水平与血压显著正相关,与新生儿出生体重显著负相关,两指标可作为妊娠性高血压患者血压升高及新生儿出生体重降低的重要预测指标。 Objective:To explore the correlations of serum miRNA-17,miRNA-181b concentration with blood pressure and pregnancy outcome in hypertensive disorder complicating pregnancy(HDCP).Method:200 patients who were hospitalized in the obstetrics department of the Second Affiliated Hospital of Xingtai Medical College from February 2016 to February 2019 were selected.According to the diagnostic criteria of the eighth edition of the Obstetrics and Gynecology textbook,the selected patients were divided into group A:early healthy pregnancy groups(n=39);group B:advanced maternal healthy pregnancy groups(n=41);group C:pregnant women with hypertension(n=49);group D:mild preeclampsia(n=41);group E:severe preeclampsia(n=30).The expression levels of miRNA-17 and miRNA-181b in the serum of each group were detected,and the blood pressure and neonatal weight of pregnancy outcomes in each group were also counted.Results:The levels of systolic blood pressure,diastolic blood pressure,urinary protein,miRNA-17,and miRNA-181b in group E were significantly higher than those of the other four groups(P<0.05);the levels of systolic blood pressure,diastolic blood pressure,urine protein,miRNA-17 and miRNA-181b in group D were significantly higher than those of group C,group B,and group A(P<0.05);the systolic blood pressure,diastolic blood pressure,urine protein,miRNA-17 levels and miRNA-181b of group C were significantly higher than that of groups B and group A(P<0.05).The rates of amniotic fluid pollution,eclampsia,fetal death,neonatal asphyxia,and intrauterine distress in group E were significantly higher than those in the other four groups(P<0.05).The proportion of neonatal birth weight was significantly lower than that of the other four groups(P<0.05).Serum miRNA-17 and miRNA-181b levels in patients with pregnancy-induced hypertension were positively correlated with blood pressure(P<0.05),and negatively correlated with neonatal birth weight(P<0.05).Conclusion:Serum levels of miRNA-17 and miRNA-181b in patients with HDCP are significantly positively correlated with blood pressure,and negatively correlated with neonatal birth weight.Both indicators can be used as important factors for increased blood pressure and neonatal birth weight reduction in patients with HDCP.
