摘要
目的分析海口市以及周边地区孕中期孕妇产前筛查唐氏综合征、18-三体综合征和神经管缺陷的产筛结果。方法选取2015年1月至2018年9月海口市以及周边地区5878例孕中期孕妇为研究对象,采用美国贝克曼公司生产的原装试剂,经贝克曼UniCel DxI 800全自动免疫分析系统检测血清甲胎蛋白(AFP)、人绒毛膜促性腺激素β亚单位(freeβ-HCG),并结合孕妇年龄、孕周、体重等采用贝克曼-腾程产筛分析软件计算,将其唐氏综合征、18-三体综合征、神经管缺陷高危比例进行统计分析,并将比较不同年龄,不同民族、城镇户口与农村户口高危比例。结果5878例孕中期孕妇中共检出唐氏综合征高风险患者436例(7.42%),18-三体综合征高风险患者9例(0.15%),神经管缺陷高风险患者100例(1.70%);年龄30~35岁和≥35岁唐氏综合征高风险高于其他年龄段,其差异有统计学意义(P<0.05),年龄≤20岁和≥35岁神经管缺陷高风险率大于其他年龄段,其差异有统计学意义(P<0.05),18-三体高风险率各年龄段差异无统计学意义(P>0.05);不同民族之间差异均无统计学意义(P>0.05);城镇户口孕妇唐氏综合征高风险率大于农村户口孕妇,其差异有统计学意义(P<0.05),农村户口孕妇神经管缺陷高风险率大于城镇户口孕妇,其差异有统计学意义(P<0.05),城镇户口孕妇和农村户口孕妇18-三体高风险率差异无统计学意义(P>0.05)。结论海口地区孕中期孕妇唐氏综合征高风险率高于神经管缺陷和18-三体综合征,不同年龄、不同户口的孕妇唐氏综合征和神经管缺陷差异比较明显,不同的人群要着重做好相应的孕期教育,干预唐氏综合征患儿的出生,从而提高我国的出生人口素质。
Objective:To analyze the screening results of prenatal screening for Down’s syndrome,18-trisomy syndrome and neural tube defects in middle pregnant women in chongzuo city and surrounding areas in southwest guangxi.Methods:Selection between January 2015 and September 2018 bay congzuo city and surrounding areas in southwest China mid 5878 cases of pregnant women as the research object,adopting the original reagent,beckman company by beckman UniCel DxI 800 full automatic immune analysis system to detect the serum AFP(AFP)and human chorionic gonadotropin beta subunits(free beta HCG),and combined with pregnant women age,gestational age,weight and so on the beckman-teng cheng sieve analysis software to calculate production,the Down’s syndrome,18-three body syndrome,analyzed at high risk of neural tube defect ratio,And will compare different age,different nationality,urban registered permanent residence and rural registered permanent residence high risk proportion.Results:a total of 436(7.42%)patients with high risk of Down’s syndrome were detected in 5878 pregnant women,9(0.15%)patients with high risk of 18-trisomy syndrome,and 100(1.70%)patients with high risk of neural tube defects.The high risk of Down’s syndrome at 30~35 years old and at or greater than 35 years old was higher than that of other ages,and the difference was statistically significant(P<0.05).The high risk rate of neural tube defects at or greater than 20 years old and at or greater than 35 years old was higher than that of other ages,and the difference was statistically significant(P<0.05).The differences among different nationalities were not statistically significant(P>0.05).Town account rate of high risk pregnant women Down’s syndrome is greater than the rural pregnant women,the difference was statistically significant(P<0.05),a rural pregnant high-risk neural tube defect rate is greater than the urban pregnant women,the difference was statistically significant(P<0.05),urban pregnant women and rural high risk pregnant women 18 to three body has no statistically significant difference(P>0.05).Conclusion:Mid pregnancy pregnant women with Down’s syndrome in southwest guangxi high risk rate is higher than neural tube defects and 18-three body syndrome,pregnant women of different ages,different account are the obvious differences in Down’s syndrome and neural tube defects,different people will focus on make corresponding prenatal education,intervention of children with down’s syndrome was born,thus improve the birth population quality in our country.
