摘要 黄褐斑是一种常见的获得性色素沉着性皮肤病,为调控皮肤色素产生及代谢的稳态机制失衡所致。传统治疗方法包括局部外用药物和化学换肤等,但由于黄褐斑的难治性和复发性,亟待新的替代治疗策略。近年来,光电技术治疗黄褐斑已在国内外得到了一定的研究和应用,其中强脉冲光、低能量调Q激光和非剥脱性点阵激光是目前最常见的光电疗法。本文系统回顾了光电技术治疗黄褐斑的有效性及安全性,简要介绍了光电技术治疗黄褐斑的作用机制,重点阐述了目前光电技术治疗黄褐斑的难点与痛点,并根据现有的治疗方案提出了策略选择建议,同时对未来光电技术治疗黄褐斑的发展进行了展望,旨在全面了解当前光电技术治疗黄褐斑的现状。 Melasma,a highly prevalent pigmented skin disease,is a dysregulation of the homeostatic mechanisms that control the metabolism of skin pigmentation.Traditional treatment approaches with topical agents and chemical peels are commonly used but due to the refractory and recurrent nature of melasma,alternative treatment strategies are huge impendence.Nowadays,laser and light therapy in melasma has been widely studied and applied.Intense pulsed light,low fluence Q-switched lasers,and non-ablative fractionated lasers are the most common lasers and light therapies that are currently performed.In this article,we systematically reviewed the efficacy and safety of laser and light therapies to treat melasma,briefly introduced the mechanisms behind the laser and light treatment in melasma,emphasized the difficulties and obstacles confronted when applying laser and light remedy to treat melasma,and provide recommendations for treatment on the basis of the reviewed information while presenting future laser-based treatment options for patients with melasma.We aim to comprehensively understand the current status of laser and light treatment in melasma.
