万古霉素的合理用药情况

Analysis of Rational Use of Vancomycin in a Hospital

目的探讨万古霉素的临床使用情况。方法回顾性分析2016年1月至2016年12月东莞市人民医院使用万古霉素的432份病历资料,对患者的一般资料、病原微生物学检查情况、药物使用情况、联合用药情况、临床疗效及不良反应进行统计分析。结果在432例患者中,呼吸内科万古霉素使用率最高(108例,25.00%),387例进行了病原微生物学检测,送检率为89.58%,万古霉素的用药频率(DDDs)为1106.8,药品利用指数(DUI)为0.82,104例为适应证不合理用药,治疗有效率为88.43%,不良反应发生率为9.49%。结论医院对万古霉素的临床应用基本合理,病原微生物学检查率合格,但也存在不合理用药情况,在万古霉素的临床使用中应严格按照规定使用,严格控制经验用药,以提高万古霉素合理用药水平,降低不良反应发生率。

Objective To explore the clinical use of vancomycin.Methods A retrospective analysis of 432 medical records of vancomycin in Dongguan People’s Hospital from January 2016 to December 2016.Statistical analysis was conducted in general information on patients,pathogenic microbiological examination,drug use,combination medication,clinical efficacy and adverse reactions of the patients.Results Among the 432 patients,vancomycin was the highest in respiratory medicine(108 cases,25.00%),and 387 cases were tested for pathogenic microbiology.The rate of detection was 89.58%,and the DDDs of vancomycin was 1106.8.DUI was 0.82.There were 104 cases of irrational use of indications,the total effective rate of clinical treatment was 88.43%,and the incidence of adverse reactions was 9.49%.Conclusion The clinical application of vancomycin in this hospital is basically reasonable,and the examination rate of pathogenic microbiology is qualified.But there are also unreasonable drug use situations.In the clinical use of vancomycin,we should strictly follow the regulations,strictly control the experience drug use,so as to improve the level of rational drug use of vancomycin and reduce the incidence of adverse reactions.
