虫类药治疗血瘀型子宫内膜异位症的研究进展

Research progress of worm drugs in the treatment of blood stasis endometriosis

子宫内膜异位症(Endometriosis,EMT)是指宫腔内具有生长功能的组织转移到宫腔黏膜以外的部位。这是一个多因素、多通路、多细胞因子共同参与的过程,其发病机制尚未明确,常表现为月经紊乱、盆腔疼痛、痛经等,严重影响患者的身心健康及生育。西医采用激素类药物,通过调节内分泌环境达到对抗或者抑制的作用,但治疗过程及预后存在不良反应,且具有复发性,导致患者依存性差。中医结合“血实宜决之”的理论,在辨证施治过程中,借虫类药的特性,行经窜络,搜经通络,从而速、猛、效地治疗EMT。现代医学发现,虫类药可通过激活自噬细胞、影响相关基因蛋白表达及下游因子含量,调控EMT发展过程中的免疫平衡-炎症效应、凝血机制成为热点。文章概述临床常用虫类药治疗血瘀型EMT的研究进展,为虫类药临床应用于痛症治疗提供理论参考。

Objective:Endometriosis(EMT)refers to the transfer of tissue with growth function in the uterine cavity to the site outside the uterine cavity mucosa.It is a process involving multiple factors,multiple pathways and multiple cytokines.The mechanism is not yet clear,and it often manifests as menstrual disorders,pelvic pain,dysmenorrhea,etc..Which seriously affects the physical and mental health and fertility of patients.Worm drugs can effectively treat EMT with promoting blood circulation and removing blood stasis,eliminating symptoms.Worm drugs can regulate the immune balance-inflammatory effect and coagulation mechanism in the development of EMT by activating autophagy cells,affecting the expression of related genes and proteins and the content of downstream factors.This article summarizes the research progress of worm drugs in the blood stasis EMT treatment,and provides a theoretical reference for the clinical application of worm drugs in the treatment of pain.
