蛛网膜下腔出血发热的中医病机

摘要 发热作为蛛网膜下腔出血的常见并发症,是增加患者不良预后发生的危险因素,若不及时控制患者的体温,发热会加重患者的神经功能缺损症状,促进脑神经元凋亡,影响整体预后。蛛网膜下腔出血引起的发热属于中医“内伤发热”的范畴,常伴有明显的精神症状或认知功能下降的症状,发热有夜热早凉的特点,且多无口渴症状,其中医病机为瘀血发热。因此我们在临床中的治疗从瘀血入手,以活血化瘀、清热凉血、开窍为治法,取得了令人满意的疗效。 As a common complication of subarachnoid hemorrhage,fever is a risk factor that increases the occurrence of poor prognosis in patients.If the temperature of patients is not timely controlled,fever will aggravate the symptoms of neurological impairment in patients,promote the apoptosis of cerebral neurons,and affect the overall prognosis of patients.The fever caused by subarachnoid hemorrhage belongs to the category of TCM internal injury fever,which is often accompanied by obvious mental symptoms or symptoms of cognitive decline.The fever has the characteristics of night heat and early cooling,and most of them have no thirst symptoms.The TCM pathogenesis is blood stasis fever.Therefore,we start with blood stasis in clinical treatment,to activate blood to remove stasis,clear heat cool blood,opening the orifical treatment,achieved satisfactory results.
