注射针刀手法三联疗法治疗腰椎间盘突出症的临床研究

A study on treating lumbar intervertebral disc herniation by triple therapy of injection plus acupotomy and manipulative reduction

目的:探索治疗腰椎间盘突出症简捷、高效、安全的优良方案。方法:精选诊断明确、适应证合理的腰椎间盘突出症患者98例,随机分为观察组50例,采用注射、针刀、手法三联疗法;对照组48例,采用针灸、牵引、中药的传统疗法。在完成治疗规程后,记录两组病例总有效率以及治疗前、第一疗次后、第二疗次后、第三疗次后和治疗结束3个月后随访的视觉模拟评分(Visual Analogue Scale,VAS)、日本骨科协会评估治疗分数(Japanese Orthopaedic Association Scores,JOA)以及腰椎Oswestry功能障碍指数(Oswestry Disability Index,ODI)的变化,评估比较观察组和对照组的疗效。结果:两组基本资料差异无统计学意义,观察组的总有效率明显优于对照组,每次治疗后观察组的VAS评分、ODI评分均低于对照组,JOA评分均高于对照组(P<0.05)。结论:注射针刀手法三联疗法在治疗腰椎间盘突出症上有较好的临床应用价值和意义。

Objective:To explore the simple,efficient and safe therapy for lumbar intervertebral disc herniation.Methods:98 cases with clear diagnosis and reasonable indications were selected and were randomly divided into two groups.50 cases in the observation group took triple therapy of injection plus acupotomy and manipulative reduction.48 cases in the control group were given traditional therapy of acupuncture plus traction and Chinese materia medica.Results:There was no statistical difference in the basic data between the two groups,and the total efficiency in the observation group was significantly better than that in the control group.The VAS score and ODI score in the observation group were lower than those in the control group,and JOA score in the observation group was higher than the control group(P<0.05).Conclusion:Triple therapy of injection plus acupotomy and manipulative reduction shows good clinical values and significances in treating lumbar intervertebral disc herniation.
