温阳利湿化瘀方对小儿胆道闭锁kassi术肠道菌群及预后的影响分析

Effect of Wenyang Lishi Huayu Recipe(温阳利湿化瘀方)on Intestinal Flora and Prognosis of Children with Biliary Atresia Kassi Operation

目的探讨温阳利湿化瘀方对小儿胆道闭锁肝门-空肠吻合术(kassi术)患儿肠道菌群及预后的影响。方法选取2018年6月—2020年6月因患胆道闭锁行kassi术的患儿60例,按照数字表法随机分为研究组和对照组,每组30例。对照组在综合治疗基础上行熊去氧胆酸片治疗,研究组在综合治疗基础上行温阳利湿化瘀方治疗,均连续治疗3月。对两组患者治疗后的肠道菌群、肠道屏障功能指标及肝功能指标等进行比较分析。结果研究组患儿的治疗有效率为90.00%(27/30),高于对照组66.67%(20/30)(P<0.05);治疗后,两组患儿的双歧杆菌、乳酸杆菌含量增多,且研究组高于对照组(P<0.05);大肠杆菌含量减少,且研究组低于对照组(P<0.05);研究组患儿治疗后的B/E值高于对照组(P<0.05);研究组患儿的血清总胆红素(TBIL)、直接胆红素(DBIL)、总胆汁酸(TBA)、碱性磷酸酶(ALP)、谷丙转氨酶(ALT)水平均低于对照组,血清蛋白(ALB)水平高于对照组(P<0.05)。结论温阳利湿化瘀方对小儿胆道闭锁kassi术患儿肠道菌群具有一定的调节作用,在增加有益菌如双歧杆菌、乳酸杆菌含量的同时可减少有害菌如大肠杆菌的含量,效果显著。

Objective To investigate the effect of Wenyang Lishi Huayu Recipe(温阳利湿化瘀方)on the intestinal flora and prognosis of children with biliary atresia kassi surgery.Methods From June 2018 to June 2020,60 children who underwent kassi surgery for biliary atresia were selected and randomly divided into study group and control group according to the number table method,with 30 cases in each group.The control group was treated with Ursodeoxycholic Acid Tablet on the basis of comprehensive treatment and the study group was treated with Wenyang Lishi Huayu Recipe on the basis of comprehensive treatment and the treatment was continued for three months.The intestinal flora,intestinal barrier function indexes and liver function indexes of the two groups after treatment were compared and analyzed.Results The effective rate of treatment of children in the study group was 90.00%(27/30),which was higher than 66.67%(20/30)in the control group(P<0.05).After treatment,the contents of bifidobacteria and lactobacilli in the two groups were increased and the contents of the study group were higher than those of the control group(P<0.05).The content of Escherichia coli was decreased and that of the study group was lower than that of the control group(P<0.05).The B/E value of children in the study group after treatment was higher than that of the control group(P<0.05).The levels of TBIL,DBIL,TBA,ALP and ALT were lower than those of the control group and the levels of ALB were higher than those of the control group(P<0.05).Conclusion Wenyang Lishi Huayu Recipe has a certain regulatory effect on the intestinal flora of children with biliary atresia kassi surgery.It can increase the content of beneficial bacteria such as bifidobacteria and lactobacillus while reducing the content of harmful bacteria such as Escherichia coli.The effect is remarkable.
