针刺鬼眼穴联合美沙拉嗪治疗伴有焦虑抑郁溃疡性结肠炎患者的疗效评估

Therapeutic effect of acupuncture at Guiyan point combined with mesalazine on ulcerative colitis with anxiety and depression

[目的]观察2种不同方案治疗伴有焦虑抑郁溃疡性结肠炎患者的临床疗效的差异性,明确针刺鬼眼穴治疗方案的有效性及对肠道黏膜屏障的影响。[方法]选择符合纳入标准的98例溃疡性结肠炎患者,随机分为治疗组和对照组,每组49例。2组均接受美沙拉嗪肠溶片(1 g口服,3次/d)治疗。对照组接受氟哌噻吨美利曲辛片治疗(每天早上1片)。治疗组接受针刺鬼眼穴治疗,疗程为4周。观察治疗前后2组患者中医症状积分、综合医院焦虑抑郁量表评分、Sutherlang疾病活动指数、Baron评分及血基质金属蛋白酶-2、血基质金属蛋白酶-9、氧化三甲胺水平的变化,并统计临床疗效。[结果]治疗组治疗后在中医症状积分、焦虑抑郁量表评分、Sutherlang疾病活动指数、Baron评分及基质金属蛋白酶-2、基质金属蛋白酶-9、氧化三甲胺水平方面明显低于对照组(P<0.05);治疗组与对照组总有效率分别为91.84%、73.47%(P<0.05)。[结论]针刺鬼眼穴联合美沙拉嗪治疗能够有效缓解伴有焦虑抑郁溃疡性结肠炎患者的临床症状、大肠黏膜病变及改善肠道黏膜屏障功能。

[Objective]To observe the difference of clinical efficacy between the two groups in the treatment of ulcerative colitis with anxiety and depression,and to clarify the effectiveness of the treatment of moxibustion on Guiyan point and its effect on intestinal mucosal barrier.[Methods]Ninty-eight patients with ulcerative colitis were randomly divided into two groups:treatment group and control group.Both groups were treated with mesalazine enteric coated tablet 1 g orally 3/day.The control group was treated with one tablet in the morning.The treatment group received acupuncture at Guiyan point.The course of treatment was 4 weeks.The changes of TCM symptom score,hospital anxiety and Depression Scale score,sutherlang disease activity index,Baron score,blood matrix metalloproteinase-2,blood matrix metalloproteinase-9,trimethylamine oxide level were observed before and after treatment,and the clinical effects were counted.[Results]After treatment,the scores of TCM symptom score,anxiety and Depression Scale(HADS),sutherlang disease activity index,Baron score,matrix metalloproteinase-2,matrix metalloproteinase-9 and trimethylamine oxide in the treatment group were significantly lower than those in the control group(P<0.05),and the total effective rate of clinical effect in the treatment group and the control group was 91.84%and 73.47%,respectively,with significant difference(P<0.05).[Conclusion]Acupuncture at Guiyan point combined with mesalazine can effectively alleviate the clinical symptoms,colorectal mucosal lesions and intestinal mucosal barrier function of ulcerative colitis patients with anxiety and depression.
