异丙托溴胺联合糖皮质激素对慢阻肺患者肺功能、炎症和氧化应激状态的影响

Effects of ipratropium bromide combined with glucocorticoids on lung function,inflammation and oxidative stress in COPD patients

目的探讨慢性阻塞性肺疾病急性加重期(AECOPD)采用异丙托溴铵与糖皮质激素联合用药治疗的效果。方法选取我院收治的AECOPD患者98例,随机分为两组,每组49例。对照组给予异丙托溴铵雾化吸入,观察组给予异丙托溴铵+布地奈德雾化吸入,比较两组肺功能、血气指标、炎症与氧化应激指标及临床有效率。结果治疗后,两组肺功能指标均高于治疗前,观察组高于对照组(P<0.05),两组炎症因子均低于治疗前,观察组低于对照组(P<0.05),观察组SOD高于治疗前,且高于对照组,MDA低于治疗前,且低于对照组(P<0.05)。观察组总有效率高于对照组(P<0.05)。结论异丙托溴铵与糖皮质激素联合用药能显著改善AECOPD患者肺功能,减轻炎症与氧化应激反应,提高治疗效果。

Objective To explore the effect of combined medication of ipratropium bromide and glucocorticoid in acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Totally 98 AECOPD patients admitted to our hospital were selected and randomly divided into two groups,with 49 cases in each group.Ipratropium bromide atomized inhalation was performed in control group,and ipratropium bromide plus budesonide atomized inhalation was performed in observation group,aiming to compare lung function,blood gas indexes,inflammation and oxidative stress indexes and clinical effectiveness between the two groups.Results After treatment,the lung function indexes in the two groups were higher than before treatment,that in the observation group was higher than the control group(P<0.05),the inflammatory factors in the two groups were lower than before treatment,and that in the observation group was lower than the control group(P<0.05).SOD in the observation group was higher than before treatment and higher than the control group,and MDA in observation group was lower than before treatment and lower than the control group(P<0.05).The total effective rate in the observation group was higher than that in the control group(P<0.05).Conclusion The combination medication of ipratropium bromide and glucocorticoids can significantly improve the lung function of AECOPD patients,reduce inflammation and oxidative stress,and improve the therapeutic effect.
