普通白光内镜与高清肠镜+i-Scan模式对结直肠息肉诊断效果对比

Comparison of the diagnostic effect of ordinary white light endoscopy and high definition colonoscopy plus i-Scan mode in the colorectal polyps

目的探究普通白光内镜与高清肠镜+i-Scan模式对结直肠息肉诊断效果,评估两种检测方式在结直肠息肉中的应用价值。方法选取于我院消化科门诊或住院的首次进行肠镜检查的患者1000例,随机分为普通白光内镜组与高清肠镜+i-Scan模式组,每组500例。比较两组息肉检出率、息肉检出数目、息肉分布特点以及组织病理学结果,统计分析两种检测方式的安全性。结果两组检查退镜时间、息肉检出率差异无统计学意义(P<0.05),高清肠镜+i-Scan模式盲肠和升结肠息肉检出数高于普通白光内镜(P<0.05),入组患者合计268枚息肉组织被送检,其中肿瘤性息肉占比57.09%,高清肠镜+i-Scan模式下检出≤5mm息肉枚数显著高于普通白光内镜(P<0.05)。结论在不增加退镜时间的基础上,高清肠镜+i-Scan模式可显著增加息肉检出总数目,尤其是对≤5mm的微小息肉,对盲肠和升结肠息肉检出数更高,肠镜检测在结直肠癌的预防诊断中具有较好的应用价值。

Objective To explore the diagnostic effect of ordinary white light endoscopy and high-definition colonoscopy plus i-Scan mode on the colorectal polyps,and evaluate the application value of the two detection methods in colorectal polyps.Methods One thousand patients who were underwent colonoscopy for the first time in the outpatient or inpatient department of the gastroenterology department of our hospital were selected,and randomly divided into the ordinary white light endoscopy group and the high-definition colonoscopy plus i-Scan mode group,500 cases in each group.Polyp detection rate,number of polyps detected,distribution characteristics of polyps and histopathological results in two groups were compared,and the safety in the two detection methods were statistically analyzed.Results There was no statistically significant difference in withdrawal time and polyp detection rate between the two groups(P<0.05).The number of polyps detected in the intestinum caecum and ascending colon in high-definition colonoscopy plus i-Scan mode was higher than that in ordinary white light endoscopy(P<0.05).A total of 268 polyp tissues were examined in the enrolled patients,of which tumorous polyps accounted for 57.09%,and the number of polyps less than 5 mm detected under high definition colonoscopy plus i-Scan mode was significantly higher than that in ordinary white light endoscopy(P<0.05).Conclusion The use of high-definition colonoscopy plus i-Scan mode without increasing the withdrawal time can significantly increase the total number of polyps detected.Relatively speaking,there is a higher number of detections for micro polyps in cecum and ascending colon polyps less than or equal to 5 mm,especially.Colonoscopy detection has good application value in the preventive diagnosis of colorectal cancer.
