心房颤动射频消融术在心脏搭桥合并心脏瓣膜手术患者治疗中的应用

Application of radiofrequency ablation of atrial fibrillation in the treatment of patients with heart bypass combined with heart valve surgery

目的探讨心房颤动射频消融术在心脏搭桥合并心脏瓣膜手术患者治疗中的应用。方法选取我院接受心脏搭桥合并心脏瓣膜手术的患者58例,随机分为两组,每组29例,对照组实施药物保守治疗,试验组实施心房颤动射频消融术。比较两组治疗前后心功能指标。结果治疗后,两组左室射血分数(LVEF)高于治疗前,左室舒张末径(LVEDD)和左室收缩末径(LVESD)低于治疗前,试验组LVEF高于对照组,试验组LVEDD和LVESD低于对照组(P<0.05),术后,两组SF-36评分优于术前,观察组优于对照组(P<0.05),干预后,两组C反应蛋白(CRP)、N末端B型脑钠肽前体(NT-proBNP)水平均优于干预前,观察组优于对照组(P<0.05)。结论心房颤动射频消融术应用在心脏搭桥合并心脏瓣膜手术患者治疗中,有助于改善心功能,促进患者康复,值得推广。

Objective To explore the application of radiofrequency ablation in the treatment of patients with heart bypass combined with heart valve surgery.Methods A total of 58 patients who underwent heart bypass combined with heart valve surgery were selected and randomly divided into two groups.29 patients in each,the reference group received conservative treatment with drugs.the control group received radiofrequency ablation for atrial fibrillation.The cardiac function indexes before and after treatment were compared between the two groups.Results After treatment,the left ventricular ejection fraction(LVEF)in the two groups was higher than before treatment,the left ventricular end diastolic diameter(LVEDD)and left ventricular end systolic diameter(LVESD)in two groups were lower than before treatment.The LVEF in the reference group was higher than that in the control group.LVEDD and LVESD were lower in reference group than in the control group(P<0.05).After the operation,the SF-36 scores in the two groups were better than those before the operation,and the reference group was better than the control group(P<0.05);C-reactive protein(CRP)and the level of N-terminal B-type pro-brain natriuretic peptide(NT-proBNP)in the two groups were better than those before intervention,and the reference group was better than the control group(P<0.05).Conclusion In the treatment of patients with heart bypass combined with heart valve surgery,the application of radiofrequency ablation for atrial fibrillation can help improve heart function and promote the recovery of patients.
