过敏性紫癜患儿血清中期因子变化的意义

目的探析血清中期因子(Midkine,MK)变化对过敏性紫癜的诊断价值。方法选择我院收治的过敏性紫癜患儿(观察组)53例,其中有21例合并有肾损害。另选取同期健康体检志愿者(对照组)37例。比较两组肿瘤坏死因子(TNF)、λ干扰素(IFN-λ)、MK、白介素2(IL-2)、IL-4、IL-6、IL-10、IL-17水平。对过敏性紫癜患儿的生化指标进行分析,包括白细胞计数(WBC)、血小板(Plt)、C反应蛋白(CRP)、血沉(ESR)、D-二聚体(D-Dimer)、免疫球蛋白A(IgA)、IgE、IgG、IgM。结果观察组MK、IL-4、IL-6、IL-17、TNF、IFN-λ水平高于对照组,IL-10低于对照组(P<0.05),两组IL-2水平比较差异无统计学意义(P>0.05)。观察组合并肾损害患儿MK高于未合并肾损害患儿,IFN-λ低于未合并肾损害患儿(P<0.05),两组其他细胞因子差异无统计学意义(P>0.05)。结论通过对过敏性紫癜患者血浆MK的检测,能够对患者病情作出早期诊断,并且可以将其作为预测肾损害的有效指标。

Objective To investigate the diagnostic value of serum midkine(MK)in children with HenochSchonlein purpura(HSP).Methods A total of 53 HSP children admitted to our hospital were selected as the research objects(observation group),of which 21 cases were complicated with renal damage.In addition,the same period of healthy volunteers were selected as the research object(control group,37 cases).TNF,IFN-λ,MK,IL-2,IL-4,IL-6,IL-10,IL-17 were compared between the two groups;the biochemical indexes of children with Henoch Schonlein purpura,including WBC,PLT,CRP,ESR,D-dimer,IgA,IgE,IgG,IgM were analyzed.Results The levels of IL-4,IL-6,IL-10,IL-17,TNF and IFN-λin the observation group were higher than those in the control group(P<0.05),while the level of IL-10 in the observation group was lower than that in the control group(P<0.05),there was no significant difference in IL-2 level between the two groups(P>0.05).The level of MK in children with renal damage in the observation group was higher than that in children without renal damage(P<0.05),the lever of IFN-λin the observation gorup is lower than that in children without renal damage(P<0.05),there was no statistically significant difference in other cytokines between the two groups(P>0.05).Conclusion For henoch schonlein purpura,through the detection of plasma MK,we can make an early diagnosis,and MK plays an important role in the development of Henoch Schonlein purpura,which can be used as an effective indicator to predict renal damage.
