针刺背俞穴联合经颅微电流刺激治疗广泛性焦虑症的临床疗效

Clinical observation on generalized anxiety disorder treated by acupuncture at back-shu points combined with TMS

目的探讨针刺背俞穴联合经颅微电流刺激(transcranial microcurrent stimulation,TMS)治疗广泛性焦虑障碍(GAD)的疗效和安全性。方法选取我院诊治的广泛性焦虑受试者60例,随机分为联合组与西药组各30例。联合组予以针刺联合TMS治疗;西药组予以枸橼酸坦度螺酮片口服治疗。观察并记录2组治疗前后汉密尔顿焦虑量表(HAMA)、焦虑自评量表(SAS)评分,血清肾上腺素(epinephrine,EPH)、5-羟色胺(5-HT)、去甲肾上腺素(NE)及多巴胺(DA)等血清神经递质水平,治疗6周后疗效及不良反应。结果联合组总有效率高于西药组(P<0.05)。治疗后,2组HAMA、SAS评分均低于治疗前,联合组低于西药组(P<0.05)。治疗后,2组的EPH、5-HT、NE、DA水平均低于治疗前,联合组低于西药组(P<0.05)。联合组的不良反应发生率低于西药组(P<0.05)。结论针刺背俞穴联合TMS治疗广泛性焦虑症临床疗效确切,可明显改善受试者症状,降低药物不良反应发生率,安全可靠。

Objective To explore the efficacy and safety of acupuncture at back-shu points combined with transcranial microcurrent stimulation(TMS)in treating generalized anxiety disorder.Methods A total of 60 patients with generalized anxiety disorder who were admitted to our hospital were divided into the combined group and the western medicine group,30 cases in each;the combined group was treated with acupuncture at back-shu points combined with TMS,the western medicine group was treated with citrate tanduluoxan tablets orally.Before and after the treatment,the scores of Hamilton Anxiety Rating Scale(HAMA)and Self-rating Anxiety Scale(SAS)and the levels of serum neurotransmitters including epinephrine(EPH),5-hydroxytryptamine(5-HT),norepinephrine(NE),and dopamine(DA)were observed and recorded between the two groups.After 6 weeks of the treatment,the curative efficacy and adverse reactions of both groups were statistically analyzed.Results The total effective rate in the combined group was higher than that in the western medicine group(P<0.05).After the treatment,the scores of HAMA and SAS and the levels of EPH,5-HT,NE,and DA in the two groups were lower than those before the treatment.And these scores and levels in the combined group were lower than those of the western medicine group(P<0.05).And the incidence of adverse reactions in the combined group was lower than that in the western medicine group(P<0.05).Conclusion The application of acupuncture at back-shu points combined with TMS has a definite clinical efficacy in treating generalized anxiety disorder,which can obviously improve the symptoms of subjects,reduce the incidence of adverse drug reactions,and be safe and reliable.
